The PAC authorization does not alter the terms, exclusions, or limitations of the underlying insurance policy. The following general exclusions apply to premium coverage and payment obligations unless otherwise expressly amended by endorsement: losses caused by fraud, intentional acts, war, nuclear hazard, or other exclusions set forth in the policy contract. Debiting of premiums under PAC does not waive any policy defenses or permit payment for coverage not otherwise afforded under the policy.

I further acknowledge that I remain bound by the terms of the insurance policy and that PAC does not alter coverage terms, premium obligations, or insurer rights.

I understand that if a debit is returned for insufficient funds or for any other reason, the insurer may attempt reinitiation and may assess applicable fees or premiums due. I release the insurer from liability for any bank charges resulting from debits made pursuant to this authorization so long as such debits are made in good faith and in accordance with the terms set forth herein.
