The Applicant authorizes the Insurer to obtain and verify records and information relevant to underwriting and claims handling, including but not limited to medical, medical billing, financial, employment, and loss history records, subject to applicable privacy laws. The Applicant further acknowledges that the Insurer may exercise subrogation rights where appropriate.

Premiums are due as stated in the Policy. Failure to pay premium when due may result in termination of coverage. Cancellation, nonrenewal, or amendment of coverage shall be effective as provided in the Policy and applicable law. Disputes arising under this Insurance Plan shall be resolved in accordance with the governing law provision below.

Acknowledgement and Certification

The Applicant certifies that the information provided in this Insurance Plan is accurate and complete. The Applicant agrees to promptly notify the Insurer of any material change in risk. Acceptance of this Insurance Plan by the Insurer is subject to final review and issuance of a policy document which will control in the event of any inconsistency.

The Applicant understands and agrees that the Insurer may cancel or modify coverage where permitted by the policy and applicable law, and that claims are subject to policy terms, limits, deductibles and exclusions.
