Underinsured Motorist Coverage Election

The named insured must elect or decline Underinsured Motorist (UIM) coverage for the policy period set forth above. Underinsured Motorist coverage provides compensation for bodily injury or death, up to the limits selected, when the at-fault motorist's liability insurance is insufficient to fully compensate the insured for covered losses. By indicating your selection below, you acknowledge receipt of the offer of UIM coverage and understand the selection is made pursuant to the terms of the policy.

If accepting coverage, select the limit option to apply for UIM benefits under this policy (select one):

Coverage Options and Coordination

Coordination of benefits: UIM benefits payable under this policy will be reduced by amounts actually paid to the insured by the tortfeasor or the tortfeasor's insurer. The insured agrees to provide the insurer with any information necessary to determine other insurance or recoveries, and to cooperate in subrogation efforts. The insurer retains the right of subrogation against any third party to the extent payments are made under this coverage.

Exclusions and Conditions

The following are material exclusions and conditions that apply to Underinsured Motorist coverage under the policy: losses arising out of a motor vehicle owned by, furnished to, or available for the regular use of the named insured where excluded by endorsement; claims subject to workers' compensation; willful or criminal acts of the insured; and any loss for which the insured has contractual indemnity from another source. Coverage is subject to all policy terms, conditions, limits, and deductible provisions.

Subrogation, Assignment and Dispute Resolution

By accepting coverage, the insured agrees that the insurer may pursue subrogation against any third party who is liable for the loss. Any recovery obtained by the insured from a third party shall, to the extent of benefits paid by the insurer, be reimbursed to the insurer after deduction of reasonable costs of recovery. Assignment of benefits is subject to policy terms and insurer approval. Disputes regarding coverage or claim amount may be resolved by alternative dispute resolution if required by the policy; acceptance of UIM benefits does not waive the insured's rights under the policy except as expressly provided herein.

Beneficiary / Settlement Payee (Optional)

Designation of a beneficiary or payee for settlement proceeds in the event of death or as otherwise permitted by the policy. This designation may be superseded by a later valid designation or by policy terms governing payable parties.
