By signing this form the Applicant warrants, represents, and covenants as follows (each a separate and material warranty):

  1. That all statements made in this form and any attachments are true, complete and correct to the best of the Applicant's knowledge and are material to the acceptance of the risk by the insurer.
  2. That the insured property and operations are maintained in a reasonably safe and prudent condition and in compliance with all applicable statutes, ordinances, building codes and regulations.
  3. That no material alteration in the risk or its exposure to loss has occurred between the date of this warranty and the inception date of the policy unless previously disclosed in writing.
  4. That the Applicant will promptly notify the insurer in writing of any change in circumstances which would render any warranty or representation untrue.
