What an Integrity Statement Template Is and Where It Fits

An Integrity Statement Template is a standardized attestation used to confirm that a person or organization will act honestly, disclose conflicts of interest, and provide accurate information for a specified engagement. Commonly used in procurement, contracting, grant administration, and internal compliance programs, the template sets out factual declarations, the scope of the attestation, and a signature block for the signer. When completed it documents intent, authorship, and the effective date of the attestation; electronic execution is generally acceptable under U.S. e-signature law (ESIGN and UETA) when intent, consent, attribution, and retention requirements are met.

Why Use an Integrity Statement Template

Using a template standardizes attestations, reduces ambiguity in vendor and employee statements, and helps organizations document compliance and due diligence. A clear template supports consistent review, auditability, and faster processing while reducing the chance of incomplete or noncompliant submissions.

Why Use an Integrity Statement Template

Who Typically Completes an Integrity Statement

Several internal and external roles commonly complete integrity statements depending on process and industry.

  • Procurement officers and contracting managers requesting vendor attestations during bid or award processes.
  • Vendors, subcontractors, and consultants certifying no conflicts of interest and accurate disclosures.
  • Compliance, legal, and HR teams requiring attestations for internal investigations, training, or onboarding.

Use the template to assign responsibility clearly and ensure every attestation is tied to a named signer, date, and record location for audit purposes.

Core Components to Include in a Professional Template

A complete Integrity Statement Template groups the attestation into clear sections so reviewers can confirm identity, scope, and signature authenticity quickly.

Heading

A concise title and version date so reviewers know which form they are reviewing; include a short description of the attestation's purpose and scope for clarity.

Parties

Full legal name of the signing individual and their organization, plus role or title; mismatched names complicate verification and may trigger additional identity checks.

Statement of Facts

Bullet or numbered statements describing the specific declarations being made (conflict disclosures, accuracy of submissions, no bribery or improper inducements).

Certifications

Formal attestations such as 'I certify under penalty of perjury' where appropriate, or a plain-language certification depending on legal requirements and risk tolerance.

Signature Block

Signature line, printed name, title, date, and contact information; include witness or notary lines when required by jurisdiction or internal policy.

Attachments

List of supporting documents (conflict disclosure forms, financial attachments, certificates) and instructions for how to append or reference them.

Step-by-Step: Completing and Submitting an Integrity Statement

Follow these sequential steps to prepare, sign, and file an integrity attestation correctly.

  • 01
Prepare the Form: Select the correct template version and gather supporting documents.
  • 02
Fill Required Fields: Enter names, dates, titles, and complete each attestation item.
  • 03
Authenticate Signer: Confirm signer identity using agreed authentication level.
  • 04
Submit and Record: Send to the designated repository and capture audit metadata.

Typical Digital Workflow Settings for Online Completion

Configure these workflow settings to match your internal approval and retention policies when moving the template online.

Field Configuration
Sender Role Compliance officer or contracting manager
Authentication Email plus optional SMS code
Attachments Allowed PDF and DOCX support
Notifications Email confirmation and audit trail logging

Where to File and Who Receives the Final Record

Routes vary by organization; capture the signed record and its audit metadata in a secure central location for future review.

  • Internal Repository: Store signed PDF in document management system.
  • Procurement Portal: Upload to the contracting or bid management portal.
  • Vendor File: Attach final attestation to the vendor record.
  • Regulatory Filing: Submit copies to regulator if required.

Technical Considerations for Digital Completion

Confirm platform and file-format compatibility before enabling online completion; consider authentication and audit-trail requirements.

  • File Types: PDF, DOCX, or HTML
  • Authentication: Email, SMS code, or stronger
  • Integrations: CRM and DMS connectors

Many platforms integrate with systems such as Salesforce, Microsoft 365, NetSuite, and cloud storage providers; ensure the chosen workflow preserves a tamper-evident audit trail and retention metadata.

Typical Timelines and Processing Expectations

Set clear timing expectations so attestations arrive when needed for procurement, audits, or contract execution.

Submission Timing:

Provide the attestation by the date requested in the solicitation or contract.

Acknowledgement:

Expect an automated receipt within 24–48 hours.

Audit Response:

Be prepared to produce originals within 30 days if audited.

Annual Renewal:

Some programs require yearly re-attestation.

Amendments:

Update attestations promptly when material facts change.

Common Mistakes to Avoid

  • Leaving name or title fields incomplete so the signer cannot be matched to corporate authority.
  • Using informal or ambiguous language in attestations that creates interpretive risk during review or audit.
  • Failing to capture required authentication or witness/notary steps where the jurisdiction or contract requires them.
  • Submitting unsigned or undated forms, which often leads to rejection and rework in procurement workflows.

Risks and Consequences of Incorrect or Missing Attestations

Contract Voidance: Award or contract may be voided
Payment Withholding: Payments can be withheld
Audit Findings: Negative audit outcomes
Tax Exposure: Possible tax or reporting impacts
Criminal Risk: False statements may trigger prosecution
Reputational Harm: Loss of trust and business
