What the Internal Audit Memo Template Is and When It’s Used

An Internal Audit Memo Template is a standardized document auditors use to summarize scope, findings, evidence, and recommended actions after a control review or audit procedure. It captures who performed the work, the period covered, the tests performed, exceptions identified, and management responses. Organizations use the template to maintain consistent audit records, support remediation tracking, and provide an audit trail for internal governance or external reviewers such as regulators, the audit committee, or external auditors.

Why a Consistent Internal Audit Memo Matters

A well‑designed template ensures completeness, supports compliance with recordkeeping rules, and speeds review by stakeholders. It reduces ambiguity in findings and helps management take measurable corrective action.

Why a Consistent Internal Audit Memo Matters

Who Typically Prepares and Reviews This Memo

The memo serves as an official internal record and a basis for follow‑up reporting to senior management and the audit committee.

  • Internal Audit — Prepares the memo, documents evidence, and assigns remediation owners.
  • Compliance / Legal — Reviews findings with regulatory or policy implications.
  • Operational Managers — Receive findings, respond with corrective action plans and deadlines.

Core Sections to Include in a Professional Memo

A complete template reduces rework and ensures reviewers can quickly understand the issue, root cause, impact, and remediation plan.

Header

Document title, unique reference number, department, author, and date for traceability and indexing purposes.

Scope

Clear description of audit objective, timeframe, systems or processes reviewed, sample sizes, and limitations encountered during testing.

Findings

Concise statement of each issue observed, including condition, criteria, cause, and effect to support a clear remediation path.

Evidence

List of working papers, screenshots, logs, and sample identifiers that substantiate each finding and can be referenced by reviewers.

Recommendations

Actionable remediation steps with assigned owners, due dates, and priority or risk rating for tracking purposes.

Management Response

Formal acceptance or rejection of findings, planned actions, timelines, and signatory acknowledgement from responsible managers.

Step-by-Step: Completing an Internal Audit Memo

Follow these sequential steps to prepare a complete, reviewable memo.

  • 01
Assemble Evidence: Gather working papers and supporting documentation first.
  • 02
Draft Findings: Write concise findings using condition, criteria, cause, and effect.
  • 03
Assign Actions: Assign owners, priorities, and specific due dates for each recommendation.
  • 04
Obtain Acknowledgement: Secure management response and signatory acknowledgement before finalizing.

Typical Routing and Approval Flow for the Memo

A standardized routing sequence ensures accountability and auditability across stakeholders.

  • Draft Prepared: Internal auditor composes memo and attaches evidence.
  • Manager Review: Operational manager reviews and provides response.
  • Audit Committee: Significant issues escalated to the audit committee for oversight.
  • Retention & Archive: Finalized memo and evidence archived per retention policy.

Frequent Preparation Pitfalls to Avoid

  • Using informal or inconsistent file names that make it hard to locate related working papers across systems.
  • Leaving action owners undefined or using group addresses; this weakens accountability and slows remediation.
  • Failing to capture supporting evidence identifiers (file names, log timestamps), which prevents verifiable follow‑up.
  • Not recording management concurrence or disagreement formally, causing disputes during external reviews.
