What the Internship Shadowing Experience Form Is

The Internship Shadowing Experience Form documents short-term observational placements where a student or prospective employee observes a professional at work. It records participant identity, host organization, dates and times, learning objectives, permitted activities, confidentiality acknowledgements, emergency contacts, and signature blocks for the participant, host supervisor, and organizational approver. The completed form creates an auditable record used for liability protection, academic credit or employer verification, and post-shadowing evaluation.

Why organizations use this form

A standardized Internship Shadowing Experience Form clarifies expectations, reduces administrative errors, documents consent and supervision, and preserves a signed record for compliance, insurer review, and academic verification.
