What an Interview Feedback Notes Is and Why It Matters

An Interview Feedback Notes document captures an interviewer’s observations, competency ratings, behavioral examples, and hiring recommendation for a specific candidate and role. It standardizes reviewer input so hiring teams compare responses consistently, documents decision rationale for compliance and audit purposes, and creates a clear record for later reference. These notes are frequently stored in applicant tracking systems, attached to candidate files, and can be completed on paper or electronically to support secure retention and controlled access during the recruitment lifecycle.

Clear benefits from consistent interview documentation

Consistent Interview Feedback Notes reduce bias, improve hiring decisions, and create defensible records for compliance and audits. Standard fields help reviewers focus on job-related criteria and preserve context for later hiring or legal review.

Clear benefits from consistent interview documentation

Typical users and where notes fit in the hiring workflow

These notes support hiring decisions, internal audits, and defense in adverse action or discrimination claims when retained and managed according to policy.

  • HR coordinators and recruiters who collect, consolidate, and file interview records for open positions.
  • Hiring managers and team leads who evaluate candidate fit and compare standardized reviewer inputs.
  • Interview panel members and subject-matter experts who complete structured assessments immediately after interviews.

Step-by-step: completing an Interview Feedback Notes

Follow these sequential actions to create complete, compliant feedback after each candidate interview.

  • 01
Open the form: Select the correct job requisition and candidate profile before starting.
  • 02
Record basics: Enter date, interviewer name, and role using MM/DD/YYYY format.
  • 03
Evaluate competencies: Rate job-relevant skills and provide concise examples of observed behavior.
  • 04
Sign and submit: Sign, date, and route to HR or upload to the ATS within established timeframes.

Essential sections that make a professional feedback note

A complete Interview Feedback Notes includes standardized sections that capture identity, assessment, evidence, and administrative metadata for reliable decision-making.

Candidate Details

Name, contact, requisition, and application ID so the note links precisely to the correct candidate record and avoids mismatches during ATS aggregation or background checks.

Interview Context

Date, interview type (phone, video, onsite), interviewer role, and interview stage to clarify where feedback fits in the hiring sequence and which competencies were evaluated.

Competency Ratings

Standardized score fields for technical skills, culture fit, communication, and problem solving that enable apples-to-apples comparisons across candidates.

Evidence & Examples

Concise behavioral examples or candidate quotes that justify each rating; specifics increase the note's credibility in later reviews or legal discovery.

Recommendation

Clear hire decision with rationale tied to role requirements and any conditions or follow-up items such as reference checks or additional interviews.

Signer Metadata

Interviewer signature, printed name, timestamp, and authentication evidence (if e-signed) to establish attribution and preserve auditability.

Tips to keep notes objective, useful, and defensible

Apply consistent practices to reduce risk, improve decision quality, and streamline review.

Use job-related language
Limit comments to observable behaviors tied to published job requirements. Avoid references to protected characteristics or subjective labels that can create legal exposure during adverse-action reviews.
Capture examples immediately
Complete notes within 24–72 hours while memory is fresh. Timely entries increase accuracy and reduce inconsistencies between reviewers.
Maintain consistent scales
Standardize rating scales across interviewers and roles. Provide calibration sessions so scores mean the same thing across reviewers and hiring managers.
Secure and centralize storage
Store notes in the ATS or secure HR system with role-based access controls and retention policies to meet audit and litigation needs.

Common pitfalls to avoid when preparing feedback notes

  • Leaving fields blank or inconsistent identifiers that prevent the note from linking to the candidate record and complicate later retrieval.
  • Using vague, subjective language instead of concrete behaviors or examples, which weakens the record in performance comparisons or legal review.
  • Failing to sign or timestamp the note, removing a key element of attribution required for audit trails and defensibility.
  • Storing notes in unsecured email or personal folders rather than the ATS, increasing risk of unauthorized access and noncompliance.

Risks if interview records are incomplete or mismanaged

Discovery Exposure: In litigation, poor records can increase liability and undermine defenses.
Discrimination Claims: Inconsistent documentation may elevate risk of adverse-action or discrimination findings.
Privacy Breach: Improper storage can trigger data-breach obligations and state privacy penalties.
Noncompliance: Failure to retain records per policy can violate internal or regulatory requirements.
Operational Delay: Missing notes slow hiring decisions and candidate communication.
Audit Failure: Incomplete audit trails impede internal and external reviews.

Time expectations for completing and processing interview notes

Set clear submission and processing timelines so feedback is timely and useful for hiring decisions and compliance.

Immediate completion:

Submit notes within 24–72 hours after the interview to preserve accuracy.

HR consolidation:

HR should consolidate panel feedback within 3 business days for decision cycles.

Candidate communication:

Communicate next steps to candidates within the timeframe defined by company policy to reduce drop-off.

Retention triggers:

Retain notes according to record-retention policy; see the retention timeline for minimum periods.

Adverse action windows:

Allow time for reference checks and review before adverse action to meet procedural fairness expectations.

Typical routing and handling workflow for completed notes

A clear routing sequence ensures notes reach decision-makers and are archived correctly.

  • Submit to ATS: Attach the completed note to the candidate record immediately after submission.
  • Notify HR: Automatically alert HR for consolidation and compliance review.
  • Share with Hiring Team: Provide hiring manager access for final decision-making and calibration.
  • Archive Securely: Move final documents to secure storage with access controls and retention tags.

Configuring online workflows for Interview Feedback Notes

Typical workflow settings ensure consistent routing, authentication, and storage when using an electronic form.

Field Configuration
Routing Rule Auto-route to HR and hiring manager after submission
Authentication Email link or SSO for interviewer identity verification
Storage Location ATS candidate file with retention tags
Notifications Email alerts for missing or late feedback

Technical requirements for digital completion and secure storage

Verify encryption, audit trails, and role-based access are in place to meet internal policy and applicable legal obligations.

  • File formats: PDF, DOCX, and HTML compatibility for ATS import
  • Integrations: Connectors for ATS, HRIS, and cloud storage (e.g., signNow, NetSuite, Google Workspace)
  • Authentication: Email/SAML and optional SMS or multi-factor authentication

Who signs and approves final interview notes

Hiring Manager

Typically reviews consolidated panel feedback, documents final decision rationale, and signs off on hire/no-hire. Their approval creates the record used for onboarding or adverse-action justification and should be authenticated and timestamped.

HR Coordinator

Collects individual interviewer notes, confirms completeness, attaches documents to the ATS, and signs or certifies the record as retained in accordance with policy and retention schedules.
