What the Interview Feedback Questionnaire Is and when to use it

An Interview Feedback Questionnaire is a standardized form used by interviewers to record observations, ratings, and recommendations after a candidate interview. It captures candidate details, competency ratings, role fit, interviewer comments, and a hiring recommendation. Organizations use the questionnaire to create a consistent evaluation record, support collaborative hiring decisions, and preserve an audit trail for compliance and internal review. When completed promptly and stored securely, it helps reduce bias, speeds decision-making, and provides documented rationale if an employment decision is later reviewed.

Why a standardized questionnaire matters for hiring quality

A consistent Interview Feedback Questionnaire improves comparability across candidates, documents decision rationale, and supports compliance with employment laws. Standard fields reduce ambiguity, help hiring teams calibrate expectations, and preserve evidence of the interview process for audits or EEOC review.

Why a standardized questionnaire matters for hiring quality

Who completes and relies on the Interview Feedback Questionnaire

Typical users include hiring managers, panel interviewers, HR staff, and recruitment coordinators who need consistent candidate records.

  • Hiring managers and interview panelists who assess role fit and competencies, and who consolidate feedback to make hiring decisions.
  • HR and people operations teams who review forms for policy compliance, track interview outcomes, and maintain personnel records.
  • Recruitment coordinators and sourcers who use standardized responses to compare candidates and communicate next steps to stakeholders.

Clear ownership for completion and review ensures the document is actionable and available for any required follow-up.

Who can sign or approve the questionnaire

Hiring Manager

The hiring manager typically completes or reviews the questionnaire and signs off on the final recommendation. Their signature or approval indicates that the hiring team considered the feedback and that the candidate assessment reflects the role requirements and interview outcomes.

HR Compliance

A human resources representative may sign or verify completion to confirm policy adherence, confirm nondiscriminatory practices, and ensure records are retained according to internal and legal retention requirements.

Essential data and protections to include

Candidate ID: Unique identifier
Interviewer Name: Printed and signed
Interview Date: MM/DD/YYYY
Role Title: Exact job title
Scores and Ratings: Standardized scale
Confidential Notes: Access-limited

Key legal and operational risks to be aware of

Discrimination claim: Insufficient documentation
Privacy breach: Unauthorized access
Inconsistent ratings: Bias exposure
Missing signature: Authentication gap
Improper retention: Policy violation
Altered records: Audit concerns

Common mistakes to avoid when issuing questionnaires

  • Delaying completion: waiting days after the interview increases memory errors and reduces the evidence value of notes for later review.
  • Using open-ended unstructured notes only: freeform responses make comparison across candidates difficult and can mask biased language.
  • Failing to secure sensitive comments: storing comments in unprotected channels risks disclosure of protected candidate information.
  • Omitting key fields such as role title or interview date: missing metadata complicates record searches and retention tracking.

How teams apply the Interview Feedback Questionnaire in practice

Two short examples illustrate common uses across hiring teams and academic admissions panels.

Tech Company Panel

A three-interviewer panel uses the same scorecard to rate technical skills and culture fit, ensuring consistent comparisons across finalists

  • Scores aggregated automatically for final ranking
  • The standardized record reduced time-to-offer and provided documented rationale for the chosen candidate during HR review and onboarding.

University Admissions

An admissions committee records interview impressions on a structured form to compare applicants to program criteria

  • Committee members rate communication and research experience
  • Centralized forms preserved reviewer comments for committee deliberations and for regulatory recordkeeping related to enrollment decisions.

Step-by-step: completing the questionnaire after an interview

Follow these steps immediately after each interview to ensure accurate, timely feedback.

  • 01
Collect candidate details: Enter full name, ID, role, and interview date.
  • 02
Score competencies: Use the provided scale for each competency area.
  • 03
Write objective comments: Note examples that support each score.
  • 04
Submit and archive: Sign, date, and send to HR for retention.

Typical workflow from form creation to storage

A straightforward four-step workflow keeps feedback efficient and auditable.

  • Create template: Design fields and scoring in a standard template.
  • Distribute link: Send to interviewers by email or internal tool.
  • Complete form: Interviewer signs and submits electronically.
  • Archive record: Store in secure HR repository with audit trail.

Core elements of a professional Interview Feedback Questionnaire

A well-designed questionnaire balances structured scoring with space for objective examples and follows privacy and retention requirements.

Standardized scorecard

Consistent rating scales for competencies reduce subjectivity and enable side-by-side comparison of candidates across interviewers and roles.

Objective evidence fields

Dedicated text fields prompt interviewers to cite observable examples that justify each rating rather than subjective impressions.

Confidential notes section

Restricted-access area for sensitive observations that HR reviews separately to manage privacy and legal risk.

Signature and timestamp

A captured signature and timestamp create an audit trail linking the evaluator to the submitted feedback.

Practical tips for accurate, defensible feedback

Apply these practices to improve quality and reduce compliance risk when collecting interview feedback.

Complete forms promptly after interviews
Finish the questionnaire within 24 to 48 hours to preserve accurate observations and to support timely hiring decisions; delayed entries increase risk of memory errors.
Use objective, behavior-based language
Describe what the candidate said or did (for example, 'explained project architecture using X pattern'), not subjective labels like 'appears arrogant' that may invite bias claims.
Limit access to sensitive notes
Restrict confidential comments to HR or designated reviewers and document who viewed or exported these notes to maintain privacy controls.
Keep consistent rating scales
Train interviewers on the same scoring rubric and provide examples to ensure scores are comparable across interviewers and roles.

Key milestones in the feedback lifecycle

Track these sequential stages from interview completion through final filing to maintain timeliness and compliance.

Detailed components to include in the questionnaire

Design fields to capture structured and narrative information that together support hiring decisions and compliance needs.

Candidate Details

Full name, candidate ID, contact details, and applied role to ensure the record links precisely to the correct application and avoids mistaken identity.

Interview Metadata

Interview date, format (phone, video, in-person), and interviewer names so HR can trace context and verify who provided the assessment.

Competency Areas

Predefined competencies with a numeric scale allow quantitative comparison across candidates and support calibrated scoring during panel reviews.

Behavioral Examples

Short text fields asking for concrete examples justify numeric scores and reduce reliance on impressionistic language.

Overall Recommendation

Clear options such as 'Proceed', 'Consider', or 'Do not proceed' guide next steps and simplify summary reporting.

Sign and Date

A signed, dated block that captures the approver or interviewer identity and provides an auditable timestamp for the record.

How to update or revise a submitted questionnaire

Follow a controlled process to correct or annotate existing feedback while preserving the audit trail.

01

Request correction:

Interviewer requests amendment via HR.
02

Document change:

Record edits with explanation.
03

HR approval:
