By issuing this Invoice Payment Summary, Issuer certifies that the amounts set forth above accurately reflect goods and services provided and payments received to date. Any dispute regarding the amounts listed must be submitted in writing within ten (10) calendar days of the Invoice Date, specifying the disputed line item and basis for dispute. Failure to timely dispute shall constitute acceptance of the recorded amounts.

Past due amounts are subject to a late charge equal to 1.5% per month (18% per annum) or the maximum rate permitted by law, whichever is less. In addition to collection of amounts due, the prevailing party shall be entitled to recover reasonable collection costs, court costs, and attorneys' fees. Returned checks are subject to a returned item fee of $35.00.
