By signing below, the Account Holder certifies that the contribution described herein is eligible for deposit to the specified IRA account, is not in excess of the applicable contribution limits for the tax year indicated, and that the Account Holder will be responsible for any tax, penalty, or corrective action required if the contribution is later determined to be ineligible.

The Account Holder represents that the contribution is being made for the tax year indicated and that, to the best of the Account Holder's knowledge, the source of funds and character of the contribution (regular contribution, rollover, transfer, conversion, or recharacterization) is correctly stated above. The Account Holder agrees to timely provide any documents or authorization requested by the Custodian to effect or substantiate the contribution.

The Account Holder authorizes the Custodian to accept and apply the contribution in accordance with this Agreement and the Custodian's customary procedures. The Account Holder understands the Custodian may reject or return contributions that are not consistent with account terms, internal policies, or applicable law.

The Custodian may accept, refuse, or return contributions in whole or in part. The Custodian is authorized to designate and report applicable tax information to the taxing authorities in accordance with law. The Custodian may charge fees for processing contributions, transfers or return of ineligible contributions in accordance with its fee schedule.

If a contribution is determined to be ineligible or excessive, the Custodian may (i) return the contribution to the Account Holder or prior plan in the manner and within the time required by law, (ii) reclassify the contribution as permitted by law, or (iii) take any corrective action necessary to comply with applicable law. The Account Holder agrees to cooperate and execute any documents necessary to effect corrective action.
