CONFIDENTIALITY

Employee acknowledges that company telephones may be used to access confidential business information. Employee agrees to protect all confidential information accessed via the job phone, to use encryption and secure credentials where required, and to immediately report any suspected compromise. Unauthorized disclosure of confidential information is grounds for disciplinary action, up to and including termination and legal remedies.

USE, LOSS AND DAMAGE

Employee accepts responsibility for reasonable care of the device. Lost or stolen devices must be reported immediately. Employee may be liable for costs arising from negligence as determined by Employer policy. Personal use beyond incidental reasonable use is subject to Employer reimbursement policy and may result in payroll deduction if authorized below.

Payroll deduction authorization: I authorize deductions for eligible device/service costs as permitted by law and company policy.

GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which Employer maintains its principal place of business. Any dispute arising under or relating to this Agreement shall be subject to the exclusive jurisdiction of the courts of that jurisdiction.

ENTIRE AGREEMENT

This document, together with any written Employer policies expressly incorporated by reference in writing, constitutes the entire agreement between the parties regarding assignment and use of the job phone and supersedes all prior understandings, representations, or agreements, whether written or oral.
