What a Jurat Page Template Is and When it’s Used

A Jurat Page Template is the standardized notary certificate attached to an affidavit, declaration, or sworn statement where the signer swears or affirms the truth of the document under oath. The jurat records the signer’s oath, the signer’s signature, the notary’s identification and credentialing, venue (state and county), the notary seal or electronic equivalent, and the notary’s signature and commission expiration. Jurats are used when a statement must be sworn before a notary public for court filings, administrative submissions, or documents that require sworn verification.

Why a Clear Jurat Page Matters

A correct jurat preserves evidentiary value by documenting oath, identity verification, and notarial acts. Clear jurat pages reduce rejection risk from courts and agencies, support chain-of-custody for sworn statements, and create a durable record whether signed in-person or via approved online notarization workflows.

Why a Clear Jurat Page Matters

Who Commonly Prepares or Signs a Jurat Page

The final jurat must reflect an in-person or approved remote notarial act, depending on state law and whether remote online notarization is used.

  • Attorneys preparing affidavits for court filings
  • Real estate professionals completing sworn statements or affidavits of title
  • Individuals signing sworn declarations for administrative or immigration processes

Essential Elements of a Professional Jurat Page Template

A professional jurat template organizes the notarial wording, signer identity, venue, date, notary signature, and registration data so the record is complete and auditable.

Jurat Wording

Standard sworn language such as "Subscribed and sworn to (or affirmed) before me" and space for the notary’s statement.

Signer Block

Clear space for the signer’s handwritten or electronic signature and printed name as it appears on identification.

Venue

State and county lines to indicate where the notarial act occurred; required for many filings.

Date

Line for the date of notarization in MM/DD/YYYY format to establish the effective notarial date.

Notary Identity

Fields for notary name, commission number, commission expiration, and any credential used for remote notarization.

Seal / Electronic Seal

Space for an impression seal or an electronic seal image plus a tamper-evident certificate when executed digitally.

Minimum Data Items to Include on the Jurat Page

Jurat Text: Full sworn statement
Signer Name: Printed name as ID
Notary Name: Full notary name
Commission Info: Commission number and expiry
Venue and Date: State, county, MM/DD/YYYY
Seal/Certificate: Physical or electronic seal

Step-by-Step: Completing a Jurat Page During Notarization

Follow these steps at the time of signing to ensure the jurat is properly executed and accepted by courts and agencies.

  • 01
Prepare Document: Attach the jurat page at the end of the affidavit or declaration
  • 02
Verify Identity: Notary examines government ID and confirms signer presence
  • 03
Administer Oath: Notary administers oath or affirmation verbally
  • 04
Complete Jurat: Signer signs, notary signs, dates, and applies seal

Where a Completed Jurat Page Typically Goes Next

After notarization, the jurat page and its parent document may be filed, recorded, or retained depending on purpose and jurisdiction.

  • Court Filing: Attach jurat to affidavits submitted with court pleadings
  • Administrative Submission: Include jurat with agency declarations or license applications
  • Recorder/Registry: If tied to a deed, file with county recorder when required
  • Document Retention: Retain original or certified copy per legal retention rules

Digital Signing and eSubmission Considerations

Choose a platform that supports secure seals, session recording for RON, and trusted storage to meet notarization and filing needs.

  • Authentication: Multi-factor or knowledge-based identity checks
  • Audit Trail: IP, timestamp, and action log retention
  • Integrations: Support for PDF, DOCX and storage systems

Timing Considerations and Common Deadlines

Jurat execution generally occurs at signing; related filing deadlines depend on the filing destination and document type.

Notarization Timing:

Complete the jurat during the same session as the oath

Court Filing Windows:

Court-specific filing deadlines vary by jurisdiction and case type

Recorder Deadlines:

Recording timelines differ by county for property-related affidavits

RON Session Retention:

Audio/visual recordings commonly retained 5–10 years depending on state

Administrative Submissions:

Agency deadlines vary; file according to the receiving authority’s schedule

Common Preparation Errors to Avoid

  • Using incomplete jurat wording or omitting the oath phrase causes rejection by courts or recorders and may require re-execution.
  • Leaving venue fields blank or using the wrong county leads to filing delays and questions about where the notarial act occurred.
  • Applying only initials or a stamp without the required notary signature and commission information undermines the jurat’s validity.
  • Attempting remote notarization where state law forbids it or without required identity proofing and session recording risks an invalid jurat.

Risks and Consequences of an Incorrect or Missing Jurat

Invalid Evidence: Court may refuse affidavit
Perjury Exposure: Signer risks criminal penalties in some contexts
Filing Delay: Rejection can delay proceedings
Contract Issues: Enforceability disputes may arise
Regulatory Penalties: Agencies may impose fines
Notary Liability: Notary can face disciplinary action

Real-World Jurat Usage Examples

These short examples show how jurats are used in common scenarios and the value of a correct template.

Court Affidavit

An attorney attaches a jurat to an affidavit for a motion

  • jurat verifies the oath and notary information
  • the court accepted the affidavit without further authentication because the jurat matched local requirements and included full notary data.
