What the Laboratory Testing Request Form Is and when it's used

A Laboratory Testing Request Form is a standardized document used to order clinical or diagnostic tests, record patient and specimen details, and authorize testing and result release. It connects ordering providers, collecting sites, and laboratory personnel, capturing identifiers (patient name, DOB, medical record number), specimen type and collection time, tests requested, clinical indications and billing information. The form supports chain-of-custody, regulatory reporting, and insurance processing. In many workflows it also records patient consent for testing and data sharing; when handled electronically the form must meet record-retention and e-signature rules applicable to healthcare and public health.

Why a clear, compliant Laboratory Testing Request Form matters

A complete request form reduces test errors, supports accurate billing, and documents consent. Electronically signed forms are legally enforceable under the federal ESIGN Act (15 U.S.C. ch. 96) and state UETA statutes (Uniform Electronic Transactions Act, 1999), though exceptions such as wills and certain court filings may apply.

Why a clear, compliant Laboratory Testing Request Form matters

Who typically completes or signs the Laboratory Testing Request Form

The form is completed and signed by people across clinical, administrative, and laboratory roles depending on the setting.

  • Ordering clinicians and nurse practitioners who specify tests, diagnosis codes, and clinical rationale for the laboratory.
  • Phlebotomists or specimen-collection staff who confirm specimen type, collection time, and labeling information at the point of collection.
  • Laboratory intake staff who verify identifiers, accession the specimen, and confirm billing and reporting instructions.

Accurate completion by the correct role speeds processing, preserves chain of custody, and reduces the need for recontact or retesting.

Step-by-step: completing the Laboratory Testing Request Form

Follow a consistent sequence to avoid common errors and to maintain specimen integrity during transport and testing.

  • 01
    1. Verify identity: Confirm full legal name and DOB match photo ID or medical record.
  • 02
    2. Select tests: Choose tests using lab codes; include CPT or test codes where required.
  • 03
    3. Record specimen details: Enter specimen type, collection date/time, collector name, and accession number.
  • 04
    4. Sign and route: Obtain ordering provider signature and route to lab with proper packaging.

Common digital workflow settings for e-submission

Configure e-submission so the form captures required fields, authenticates signers, and routes automatically to the laboratory information system.

Field Configuration
Signer authentication Email link with optional SMS code for higher assurance
Field validation Required fields enforced; date format MM/DD/YYYY
Auto-routing Send completed orders to LIS via secure integration
Audit trail capture Timestamps, IP, and signer identity logged

Digital sharing and eSubmission: platform considerations

Choose platform settings that preserve data security, meet healthcare compliance, and integrate with your lab systems.

  • Integrations: Supports HL7/LIS, API, or CSV exports
  • File formats: Accepts PDF, DOCX, and structured data feeds
  • Authentication: Email, SMS, or stronger methods available

For clinical workflows, prioritize platforms with HIPAA support and audit trails. Confirm integration options with your laboratory information system before rollout.

How electronic ordering and signing typically flow

An efficient e-ordering workflow reduces manual handoffs and supports faster laboratory turnaround times.

  • Upload order: Sender uploads the completed request form to the signing platform.
  • Place fields: Add required signature, date, and checkbox fields for consent and tests.
  • Notify signer: System emails or texts a secure signing link to the provider or patient.
  • Send to lab: Signed form and audit trail route to the laboratory intake or LIS.

Essential components of a professional Laboratory Testing Request Form

A well-crafted form balances needed clinical detail, administrative data, and legal elements to support accurate testing and reporting.

Patient identification

Includes full legal name, date of birth, and a unique medical record or patient ID so results map unambiguously to the correct individual and chart.

Clinical indication

A concise clinical diagnosis or reason for testing, including relevant signs or symptoms, helps laboratories select correct panels and interpret results when reflex testing is needed.

Test codes and CPT

Clear test names with laboratory-specific test codes and CPT codes ensure accurate billing and reduce coding disputes with payers.

Specimen details

Specimen type, collection date/time, collector name, and any special handling instructions are recorded to preserve sample integrity and valid results.

Consent and release

Explicit consent statements for testing and data release, including parental consent for minors where required, protect legal compliance and patient rights.

Billing and payer info

Insurance payer, subscriber ID, and billing party details are included to facilitate claims processing and reduce billing delays.

Security and compliance elements to include

Encryption: TLS 1.2/1.3 and AES-256
HIPAA: BAA required for PHI handling
Audit trail: Time, IP, and action logs
Access controls: Role-based permissions
21 CFR Part 11: Required for FDA-regulated records
SOC 2: Evidence of operational controls

Consequences of incomplete or incorrect forms

Patient safety: Risk of missed or delayed diagnosis
HIPAA violations: Civil penalties possible (45 CFR §§160–164)
Specimen mislabeling: Rejected or mismatched results
Billing denials: Claims refused for missing codes
Legal exposure: Malpractice and regulatory risk
Chain-of-custody: Forensic evidence integrity affected

Common mistakes to avoid when preparing the form

  • Using nicknames or initials instead of the patient’s full legal name leads to specimen misidentification and delays in reporting.
  • Leaving collection date/time blank prevents proper interpretation of time-sensitive assays and may invalidate test results.
  • Omitting payer or CPT information causes billing denials and increases administrative follow-up for claims adjudication.
  • Failing to capture explicit consent when required for sensitive tests (for example, HIV) can create regulatory and legal exposure.

Typical processing timelines and reporting expectations

Turnaround and reporting expectations vary by test priority and regulatory obligations; plan collections and submissions accordingly.

Stat/urgent tests:

Results commonly within hours for STAT-designated assays

Routine testing:

Typical 24–72 hour turnaround depending on assay complexity

Public health reporting:

Reportable diseases must be reported to health departments immediately under state law

Specimen discard window:

Many labs discard untested specimens after 7–14 days

Consent revocation:

Document any withdrawal promptly to stop nonessential testing

Key milestones from order to results

Real-world examples of form use

Two brief scenarios show how a properly completed form speeds care and preserves compliance.

Hospital outpatient ordering

A clinician completes an electronic request with structured fields and e-signature

  • STAT label triggers expedited processing
  • The lab receives clear specimen details and returns critical results within hours, reducing length of stay and follow-up calls.

Clinical research enrollment

A study coordinator captures consent and testing orders via an audited electronic form

  • Versioned consent links to the request for traceability
  • This preserves IRB compliance, links samples to protocol IDs, and simplifies sponsor reporting.
