What the Laundry Service Wash and Dry Form Is

The Laundry Service Wash and Dry Form is a standardized service order and receipt used by laundromats, pickup-and-delivery services, and uniform suppliers to capture customer instructions, item counts, special handling requests, pricing, and signature authorization. It documents the services requested (wash, dry, fold, dry-clean), identifies items that require special treatment (delicate, stain treatment, permanent press), records any customer-supplied accessories or valuables, and creates an audit trail for pickup, processing, and return. The form can be printed or completed electronically and is useful for customer disputes, billing reconciliation, and chain-of-custody tracking.

Why use a formal Wash and Dry Form

A clear, consistent form reduces errors, supports accurate billing, and documents customer instructions and liability limits in case of damage or loss.

  • Omitting item counts or combining items in one line can cause billing disputes and missing-item claims.
  • Using unclear pricing terms or not recording rate basis (per pound vs per item) may result in unexpected charges.
  • Not obtaining a signature or recorded consent for special handling voids the strongest evidence in liability disputes.

Risks and liability language to include

Declared Value: Limit liability per item
Damage Claims Window: File within specified days
Exclusions: Exclude pre-existing damage
Accessory Disclaimer: No liability for missing small items
Refund Terms: Define refund conditions
Arbitration: Consider dispute resolution clause

Operational best practices for accurate forms

Adopt these practices to reduce errors, speed processing, and strengthen customer trust.

Standardize item categories
Use a fixed list of garment types and abbreviations on every form. Consistency reduces miscounts, simplifies inventory reconciliation, and enables automated pricing calculations in POS or workflow systems.
Photograph high-value items
Document pre-existing stains or damage with photos attached to the order. Photo evidence significantly reduces dispute resolution time and supports liability decisions when items are claimed damaged after service.
Confirm pricing basis up front
Clearly mark whether pricing is per pound, per item, or flat-rate. Obtain customer acknowledgement of the pricing method to reduce surprise charges and chargebacks, especially for pickup-and-delivery orders.
Keep electronic receipts
Provide customers an emailed or texted copy of the signed form. Electronic receipts preserve timestamps and audit trails and are easier to search for returns or claims.
