What the Layout Request Form Is and when it’s used

The Layout Request Form is a standardized document used to collect requirements, assets, schedule, and approval details for a print or digital layout project. It centralizes technical specifications (trim size, bleed, color profile), required deliverables, contact information, and milestone dates so designers, production teams, and vendors have a single authoritative source for work instructions and acceptance criteria.

Why a clear request form matters

A well-constructed Layout Request Form reduces rework, shortens production cycles, and clarifies ownership by documenting scope, deadlines, and file requirements before design begins.

Why a clear request form matters

Typical teams and roles that complete this form

This form is used by anyone who initiates, approves, or executes layout work across marketing, creative services, and vendor management.

  • Marketing managers requesting promotional assets, providing campaign context and final deliverable expectations.
  • Graphic designers and production artists receiving technical specifications, file formats, and asset references to start layout work.
  • Vendors or printers needing trim, bleed, color profile, and proofing instructions to produce physical or digital outputs.

Use the form to set expectations, attach source files, and record signatory approval to avoid downstream disputes or delays.

Primary signers and approvers

Requestor — Marketing Manager

The requestor completes project context, target audience, and business objectives, attaches source assets, and confirms budget and deadline. Their approval certifies requirements are accurate before work begins.

Approver — Creative Director

The approver verifies design brief alignment, confirms technical specs and legal clearances (copyright, trademarks), and signs to release production. Their signature triggers final file preparation and handoff to vendor.

Essential sections to include in a professional form

A complete Layout Request Form groups scope, technical data, schedule, assets, approvals, and delivery instructions so teams can act without additional clarification.

Project Scope

Describe purpose, audience, and required outputs (e.g., brochure 8.5x11 double-sided PDF; web hero 1920x1080 JPG). Be specific about language, versions, and variants needed.

Technical Specifications

List trim size, bleed, safe area, DPI, color space (CMYK/RGB), fonts, and file formats (PDF/X-1a, INDD, EPS). Include required export settings for print or digital.

Milestones & Deadlines

Define request date, first proof due, revision windows, and final sign-off date. Tie each milestone to a responsible contact and time zone.

Assets and References

Attach logos, images, copy decks, contracts, and copyright licenses. Note acceptable file naming conventions and master file ownership.

Acceptance Criteria

Specify what constitutes approval (visual proof, color proof, PDF preflight, or print sample) and any quality checks required prior to production.

Signatures & Payment

Include signature block for requestor and approver, plus payment terms or purchase order reference when vendor work requires invoicing.

Step-by-step: completing and routing the form

Follow these steps to submit a complete request and move the layout into production without avoidable delays.

  • 01
    1. Prepare assets: Gather high-resolution logos, copy, and any legal approvals before beginning.
  • 02
    2. Complete fields: Fill all required fields, include dimensions, color profile, and delivery date.
  • 03
    3. Attach files: Upload source files and name them per convention for easy tracking.
  • 04
    4. Route for approval: Assign approvers in order and record expected review windows.

How to configure an online layout request workflow

Set up workflow options that match your organization’s review and approval process to automate routing and notifications.

Field Configuration
Required Fields Mark Project Title, Delivery Date, and Attachments as required.
Signer Order Set sequential or parallel approvals based on internal policy.
Conditional Fields Show additional input when 'Print' versus 'Digital' is selected.
Notifications Enable email and in-app reminders for pending approvals.

Where to send the completed Layout Request Form

Route the completed form to internal stakeholders and external vendors using a defined channel so responsibilities are clear.

  • Internal Review: Send to Creative Director or assigned approver for validation and sign-off.
  • Vendor Handoff: Share final approved assets and production-ready PDFs with the printer or developer.
  • Project Tracking: Log the form in your project management tool for milestone monitoring.
  • Archive: Store approved assets and final sign-off for quality audits and re-orders.

Sharing and eSubmission options for the Layout Request Form

The form can be distributed by email, cloud links, or integrated workflows depending on security and tracking needs.

  • Email with attachment: Simple delivery; lower tracking capability and higher risk of versioning errors.
  • Cloud storage link: Use Google Drive, Box, or Egnyte for large assets and controlled access.
  • Integrated workflow: Connect to CRM or project tools (Salesforce, NetSuite, Microsoft 365) for automated routing.

Select the method that balances convenience with required auditability and access controls for your project and industry.

Security and compliance considerations for electronic submission

Encryption: TLS 1.2/1.3; AES-256 at rest
Certifications: SOC 2 Type II; ISO 27001
HIPAA: BAA available for protected health data
Audit Trail: Timestamps, IP, and action logs
21 CFR Part 11: Compliant options for FDA records
Access Control: SSO, role-based permissions

Consequences of errors or incomplete requests

Missed Deadline: Production delay
Wrong Specs: Print waste, rework
Unlicensed Assets: Copyright claims
Incomplete Approval: Order cancellation
Incorrect Contact: Failed delivery
Security Lapse: Unauthorized access

Common mistakes that slow layout production

  • Submitting low-resolution images that cannot be used for print, requiring last-minute asset retrieval and delays.
  • Omitting color profile or bleed settings, causing print mismatches and additional proofing rounds.
  • Unclear or changing approval lists, leading to repeated sign-off loops and missed launch dates.
  • Not attaching copyright/usage licenses for third-party assets, exposing the project to legal takedowns.

Practical tips to complete the form accurately and efficiently

Adopt these practices to reduce back-and-forth and ensure deliverables meet technical and legal requirements.

Standardize naming and formats
Use consistent file naming conventions, preferred file types, and a template checklist to prevent version confusion and speed processing.
Attach source files and licenses
Provide original vector logos, layered files, and written license or release documents to avoid legal issues and minimize revisions.
Set realistic timelines
Account for proofing cycles and vendor lead times when selecting delivery dates, and communicate cutoff times for change requests.
Include acceptance criteria
Define what constitutes final approval (signed PDF, digital certificate, or printed proof) to eliminate ambiguity at handoff.

Real-world examples of using a Layout Request Form

These examples show how standardized requests simplify collaboration between requestors, designers, and external vendors.

Martin Properties — Marketing Collateral

A property management firm standardized requests to reduce turnaround time.

A large enterprise integrated requests with its ERP for routing.

  • Automated field detection and signatory routing saved manual steps.
  • This approach ensured correct file formats reached vendors and maintained compliance with corporate procurement and NetSuite workflows.

Typical timeline expectations and internal deadlines

Set internal milestones to coordinate design, reviews, and production so stakeholders share a common schedule.

Submission Deadline:

Request submitted at least 14 days before launch

Initial Proof:

Designer provides first proof within 3 business days

Revision Window:

Each revision cycle should be limited to 2 business days

Final Approval:
