What the Legal Client Intake Packet Is

The Legal Client Intake Packet is a standardized collection of forms and prompts used by law firms and in-house legal teams to gather client identity, matter facts, conflict information, fee terms, and supporting documents at the outset of an engagement. It centralizes contact details, scope of representation, retainer terms, conflict disclosures, and consent language so staff can perform conflict checks, set up matter files, and document client authority. Packets are often tailored for practice area needs and may include tax, HIPAA, or notarization fields where applicable.

Why a Standard Intake Packet Matters for Firms

A structured intake packet reduces onboarding delays, improves conflict screening, and creates an auditable record of client consent and fee arrangements. It supports regulatory compliance, consistent billing setup, and more efficient matter opening across attorneys and offices.

Why a Standard Intake Packet Matters for Firms

Who Typically Completes and Reviews the Packet

Firms, solos, corporate legal departments, and intake teams collect the packet information during initial client intake or pre-engagement checks.

  • Law firms — Intake coordinators collect core client and matter details for conflict and file setup.
  • In-house legal teams — Paralegals and counsel gather vendor or contract counterparty data for matters.
  • Solo practitioners — Attorneys use a compact packet to confirm scope, fees, and client authority quickly.

Different roles use the packet to confirm identity, perform conflicts checks, set billing, and document authorization before work begins.

Primary Users and Roles

Managing Partner

Uses the packet to ensure conflicts checks and retainer terms meet firm policy, and to approve nonstandard fee arrangements or high-risk matters.

Intake Coordinator

Completes initial client contact, collects identity documents and supporting forms, routes the packet to billing and assigned counsel, and keeps the audit trail for compliance.

Essential Sections to Include in a Professional Packet

A complete packet covers both administrative and legal checkpoints to reduce onboarding friction and regulatory risk.

Client Identity

Full legal name, date of birth or EIN for entities, government ID references, and contact information to verify authority and run KYC or identity checks.

Matter Summary

Concise description of the legal matter, jurisdiction, desired outcome, and key dates so the firm can determine scope and resource allocation.

Conflict Disclosure

Questions about related parties, adverse interests, and prior representations plus signature acknowledging disclosure truthfulness for conflict clearance.

Fee and Retainer Terms

Standardized fee structure, billing rates, estimated costs, retainer amount, and agreement to terms to reduce billing disputes later.

Supporting Documents

Requested documents such as corporate formation records, contracts, identification, HIPAA authorizations, or financial statements relevant to the intake.

Signature & Consent

Signature blocks for client acceptance, electronic consent language (if e-signed), and optional authorization for credit or background checks.

Security and Compliance Controls to Note

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Audit Trail: Timestamped actions, IP, and signer attribution
HIPAA Capability: BAA available where required
Regulatory Standards: Supports ESIGN, UETA, 21 CFR Part 11

Common Mistakes to Avoid When Preparing Intake Packets

  • Leaving name or entity inconsistencies between ID and contract which can block payments or create tax reporting issues.
  • Failing to include conflict-relevant names or related parties, producing late disqualifications or ethical investigations.
  • Omitting required consents or disclosures for consumer-facing financial or healthcare matters, risking ESIGN consumer disclosure noncompliance.
  • Not verifying signer authority for corporate clients, resulting in unenforceable agreements or later repudiation.

Risks and Potential Consequences

Tax Withholding: Backup withholding risk with invalid TINs
I-9 Violations: Penalties per form for noncompliance
Conflict Exposure: Disqualification or malpractice claims
HIPAA Breach: Regulatory penalties and remediation costs
Invalid Signature: Enforceability challenges in court
Data Loss: Client confidentiality and reputational harm

Practical Tips for Faster, More Accurate Intake

Implement these practices to streamline intake and reduce downstream work.

Use a single master template
Maintain one authoritative intake template version to avoid divergent workflows and ensure consistent conflict screening and billing setup across the firm.
Leverage conditional fields
Show only relevant sections (e.g., HIPAA release for healthcare matters) to shorten signer experience and reduce input errors.
Automate routing
Automatically send completed packets to assigned counsel, billing, and records storage to reduce manual handoffs and speed matter opening.
Validate critical fields
Use automated checks for TIN formats, email addresses, and required signatures to prevent avoidable rework and compliance failures.
