Recitals

2. Disclosure Requirement

The Respondent must disclose, in writing, all actual, potential, or perceived Conflicts of Interest that arise or are known to the Respondent as of the effective date of this Questionnaire or at any time during the Respondent's relationship with the Company. Disclosures must be complete and include the nature, extent, and materiality of the Interest and any steps taken to mitigate the interest.

7. Remedies and Enforcement
