LEGAL CREDIT CONSENT FORM

RECITALS

WHEREAS, the Requesting Party requires authorization to obtain consumer credit information and related verification concerning the Consumer to evaluate creditworthiness, eligibility for services or products, or to otherwise process an application submitted by the Consumer; and

WHEREAS, the Consumer is willing to provide such authorization subject to the terms and conditions set forth in this Consent Form, including limitations on scope, duration, and permitted uses; and

WHEREAS, the Parties intend by this Consent Form to satisfy applicable notification and authorization requirements for the disclosure and use of consumer credit reports and associated information.

NOW, THEREFORE

In consideration of the mutual covenants contained herein and other good and valuable consideration, the Parties agree as follows:

1. AUTHORIZATION

The Consumer expressly authorizes the Requesting Party, its agents, assigns and third-party service providers, to obtain and use consumer reports, investigative consumer reports, payment history, employment and income verification, tenant screening reports and other credit-related information (collectively, "Credit Information") about the Consumer from credit reporting agencies and other third parties for the purposes of evaluating eligibility for credit, employment, tenancy, insurance or other lawful business purposes specified by the Requesting Party.

2. SCOPE AND PERMITTED USES

The authorization granted shall extend to obtaining Credit Information in connection with: (a) credit underwriting or account management; (b) verifying identity, employment, income or residency; (c) risk assessment and fraud prevention; and (d) any other legitimate business purpose as permitted by law. The Requesting Party shall not use Credit Information for purposes prohibited by applicable law.

3. DURATION

This Consent shall become effective on the Effective Date set forth above and shall continue for a period of unless earlier revoked in writing in accordance with Section 8. The Requesting Party may, at its discretion, request reauthorization after expiration.

4. DISCLOSURES AND ACKNOWLEDGMENTS

The Consumer understands that: (a) Credit Information may be obtained from one or more consumer reporting agencies; (b) such Credit Information may include public records, payment history, account balances and inquiries; and (c) adverse actions based on information obtained may be taken, provided the Consumer is afforded any rights required by law.

5. LIMITATIONS ON DISCLOSURE

The Requesting Party shall use reasonable administrative, technical and physical safeguards to protect Credit Information against unauthorized access and shall disclose Credit Information only to third parties strictly necessary to carry out the permitted purposes described herein, including service providers acting on behalf of the Requesting Party, and only under confidentiality obligations.

6. REPRESENTATIONS AND WARRANTIES

Each Party represents and warrants that it has full authority to enter into this Consent and that its execution, delivery and performance will not violate any agreement or law applicable to such Party. The Consumer represents that the identifying information provided is true and accurate to the best of Consumer's knowledge.

7. INDEMNIFICATION

The Requesting Party shall indemnify and hold harmless the Consumer from any claims arising from the Requesting Party’s negligent or willful misuse of Credit Information. The Consumer shall indemnify and hold harmless the Requesting Party from any claims arising from material misrepresentations by the Consumer in connection with this Consent.

8. REVOCATION

The Consumer may revoke this Consent at any time by providing written notice to the Requesting Party in accordance with the Notices section below. Revocation shall not affect disclosures made prior to receipt of the revocation notice and shall not invalidate actions taken in reliance on Credit Information obtained while this Consent was effective.

No amendment or waiver of any provision of this Consent shall be effective unless in writing and signed by both Parties. The failure of a Party at any time to require performance of any provision shall not affect that Party's right to require performance at any time thereafter.

11. GOVERNING LAW; VENUE

This Consent shall be governed by and construed in accordance with the laws of the state indicated below without regard to conflict of laws principles. The Parties submit to the exclusive jurisdiction of the state and federal courts located in the county identified below for purposes of any action arising out of this Consent.

12. ENTIRE AGREEMENT; SEVERABILITY; COUNTERPARTS
