4.2 Verification. Where verification or affidavit is required by rule or statute, Filer shall attach a separate verified statement or affidavit signed before a person authorized to administer oaths and shall deliver a copy to Respondent contemporaneously with submission to the filing office.

5. SERVICE AND NOTICE

5.1 Service Methods.
