LEGAL INTAKE PACKET

This Legal Intake Packet (the "Agreement") is entered into as of by and between Client Name: with primary address: ("Client"), and Firm Name: with principal place of business: ("Firm").

RECITALS

WHEREAS, Client seeks legal representation and advice concerning the matter described below; and

WHEREAS, Firm is duly qualified and willing to provide legal services to Client in accordance with the terms and conditions set forth in this Agreement; and

WHEREAS, the parties desire to set forth their respective rights, duties, and obligations with respect to the representation.

NOW, THEREFORE, in consideration of the mutual promises contained herein and other good and valuable consideration, the parties agree as follows:

1. ENGAGEMENT AND SCOPE

1.1 Engagement. Client retains Firm to provide legal services relating to the matter described as:

1.2 Scope Limitations. The scope of representation is limited to the tasks expressly authorized by Client in writing. Firm will not provide services outside the scope unless the parties agree in writing.

2. CLIENT RESPONSIBILITIES

Client shall cooperate with Firm, provide truthful and complete information, produce records and documents reasonably requested, and notify Firm of all material developments. Client acknowledges that failure to cooperate may lead to termination of representation.

3. FEES, RETAINER, AND BILLING

3.1 Fees. Unless otherwise agreed in writing, Client will be billed at the following rates:

3.3 Billing and Payment. Firm will render periodic invoices setting forth fees, costs, and disbursements. Payment is due upon receipt. Past-due balances may incur interest at the maximum rate permitted by law and may result in suspension or termination of services.

4. COSTS AND EXPENSES

Client is responsible for all out-of-pocket expenses incurred by Firm on Client's behalf, including but not limited to filing fees, courier charges, expert fees, deposition costs, travel, and photocopying. Firm may require payment of anticipated expenses in advance.

5. CONFLICTS AND CONSENT

Firm has conducted a conflicts check based on information provided by Client. Client represents that the information supplied is complete and accurate. If a conflict exists or subsequently arises, Firm will notify Client promptly and may be required to withdraw unless Client provides informed written consent.

6. CONFIDENTIALITY AND PRIVILEGE

Communications between Client and Firm made for the purpose of seeking or rendering legal advice are protected by the attorney-client privilege and will be treated as confidential except as authorized by Client or required by law. Client authorizes Firm to disclose confidential information only (a) as necessary to carry out representation, (b) with Client's consent, or (c) as required by court order or law.

7. RECORDS AND FILE RETENTION

Firm will maintain files and records related to the representation. Upon conclusion of the matter, Firm may retain copies and discard original materials after providing Client with notice. Client may request delivery of original documents; Firm may charge reasonable copying and handling charges.

8. TERMINATION

Either party may terminate this Agreement at any time upon written notice. Upon termination, Client shall pay all earned fees and incurred costs through the date of termination. Firm may retain files and documents until outstanding balances are paid, to the extent permitted by law.

9. NOTICES

All notices required or permitted by this Agreement shall be in writing and delivered to the addresses below by hand, certified mail, or a nationally recognized overnight courier, or by email with confirmation of receipt.

10. AUTHORIZATIONS

Client authorizes Firm to: (a) obtain and review records from third parties as necessary for representation; (b) communicate with opposing counsel and third parties; and (c) take such actions as are reasonably necessary to protect Client's interests. Client authorizes Firm to charge the retainer/trust account for fees and costs in accordance with this Agreement.

11. GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY

This Agreement shall be governed by and construed in accordance with the laws of the state specified by the Firm's principal office unless otherwise agreed in writing. This Agreement constitutes the entire agreement between the parties relating to the subject matter hereof and supersedes any prior oral or written agreements. If any provision of this Agreement is determined to be invalid or unenforceable, such provision shall be severed to the minimum extent necessary and the remaining provisions shall continue in full force and effect.

12. AMENDMENT; WAIVER; COUNTERPARTS

No amendment to this Agreement will be effective unless in writing and signed by both parties. The failure of either party to enforce any right shall not constitute a waiver of that right. This Agreement may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one instrument.

13. ACKNOWLEDGMENT

By signing below, Client acknowledges receipt of this Legal Intake Packet, accepts the terms and conditions set forth herein, and authorizes Firm to proceed with representation in accordance with this Agreement.
