LEGAL MINOR ANNEX

RECITALS

2. PARENTAL CONSENT AND AUTHORIZATION

The Parent or Legal Guardian represents and warrants that they are the lawful parent or legal guardian of the Minor and have full authority to enter into this Annex. The Parent or Legal Guardian hereby consents to the Minor's participation in the Activities and authorizes Party A and its agents to permit the Minor to engage in such Activities subject to the terms of the Principal Agreement and this Annex.

3. MEDICAL TREATMENT AUTHORIZATION

The Parent or Legal Guardian hereby authorizes Party A to obtain, and to consent to, emergency medical or surgical treatment for the Minor if warranted, including hospitalization, anesthesia, or other medical procedures, if the Parent or Legal Guardian cannot be reached after reasonable attempts. The Parent or Legal Guardian agrees to be financially responsible for any such treatment and indemnifies Party A for reasonable costs incurred.

4. RELEASE, WAIVER AND ASSUMPTION OF RISK

The Parent or Legal Guardian, on behalf of the Minor and themselves, hereby releases, waives, and discharges Party A and its officers, directors, employees, agents, contractors and affiliates (collectively, "Released Parties") from any and all liability, claims, demands, causes of action, suits or losses arising out of or related to the Minor's participation in the Activities, including claims for personal injury, death or property damage, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

5. INDEMNIFICATION

The Parent or Legal Guardian agrees to indemnify, defend and hold harmless the Released Parties from and against any and all losses, liabilities, damages, claims, costs and expenses (including reasonable attorneys' fees) arising out of or resulting from the Minor's acts or omissions or any breach of this Annex by the Parent or Legal Guardian.

6. MEDIA AND PUBLICITY RELEASE

The Parent or Legal Guardian grants Party A the irrevocable right to photograph, videotape, record and use the Minor's image, likeness, voice and performance in connection with the Activities for promotional, marketing, or educational purposes worldwide and in perpetuity, unless the Parent or Legal Guardian opts out below.

7. CONFIDENTIALITY

9. NOTICES

This Annex, together with the Principal Agreement, constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements and understandings. If any provision of this Annex is held to be invalid or unenforceable, that provision shall be modified to the minimum extent necessary to make it enforceable, or if modification is not possible, it shall be severed, and the remainder of this Annex shall remain in full force and effect.
