LEGAL MINOR WAIVER

This Legal Minor Waiver (the "Waiver") is made effective as of by and between Parent/Guardian Name: of Address: ("Guardian"), on behalf of Minor: Date of Birth: , and Organizer/Operator: of Address: ("Organizer").

RECITALS

WHEREAS, Guardian is the parent or legal guardian authorized to execute this Waiver on behalf of the Minor identified above; and

WHEREAS, the Minor will participate in the activity described as to be held on or about at ; and

WHEREAS, Guardian and Organizer desire to define their respective rights and responsibilities with respect to the Minor's participation in the activity.

NOW, THEREFORE, in consideration of the mutual covenants contained herein and other good and valuable consideration, the receipt and adequacy of which are hereby acknowledged, the parties agree as follows:

1. DEFINITIONS

"Activity" means the event, program, or services described in the Recitals and any ancillary activities, transportation, practice, or training related thereto. "Claims" means any and all claims, demands, causes of action, damages, losses, liabilities, costs and expenses (including reasonable attorneys' fees) whether known or unknown, foreseen or unforeseen.

2. ASSUMPTION OF RISK

Guardian acknowledges that the Activity involves inherent risks, including but not limited to personal injury, permanent disability, death, and property loss. Guardian voluntarily assumes all risks associated with the Minor's participation in the Activity, whether caused by the negligence of Organizer, other participants, or otherwise, to the fullest extent permitted by law.

3. RELEASE AND WAIVER

To the fullest extent permitted by applicable law, Guardian, on behalf of the Minor, hereby releases, discharges, and covenants not to sue Organizer and its officers, directors, employees, agents, volunteers, contractors and affiliates (collectively "Released Parties") from and for any and all Claims arising out of or related to the Minor's participation in the Activity, including but not limited to Claims resulting from the negligence of the Released Parties.

4. INDEMNIFICATION

Guardian agrees to indemnify, defend and hold harmless the Released Parties from and against any and all Claims arising out of or related to (a) the Minor's acts or omissions, (b) Guardian's breach of this Waiver, or (c) Guardian's negligent supervision of the Minor, including payment of reasonable attorneys' fees and costs incurred in defense of any claim.

5. MEDICAL AUTHORIZATION

In the event of illness or injury to the Minor, Guardian authorizes Organizer and its representatives to secure emergency medical treatment for the Minor and agrees to be financially responsible for any medical services rendered. Guardian acknowledges that Organizer may not have medical personnel on site and that any decision to seek medical care will be made in Organizer's discretion when necessary.

6. PHOTO, VIDEO, AND MEDIA RELEASE

Guardian grants Organizer the unrestricted right to use, reproduce, and publish photographs, video, or audio recordings of the Minor taken in connection with the Activity for promotional, educational or commercial purposes, without further consent or compensation, unless Guardian checks the box below to withhold consent.

7. REPRESENTATIONS AND WARRANTIES

Guardian represents and warrants that: (a) Guardian is the parent or legal guardian of the Minor and has full authority to enter into this Waiver; (b) all information provided in this Waiver is true and complete; and (c) Guardian will immediately notify Organizer in writing of any change in the Minor's physical condition that may affect participation.

8. TERM

This Waiver shall be effective as of the Effective Date set forth above and shall survive the Minor's participation in the Activity and continue in perpetuity with respect to any Claims that may arise.

9. NOTICES

All notices required or permitted under this Waiver shall be in writing and addressed as follows:

This Waiver shall be governed by and construed in accordance with the laws of the state identified below, without regard to conflict of law principles.

11. ENTIRE AGREEMENT

This Waiver constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings, whether oral or written.

12. SEVERABILITY

If any provision of this Waiver is held to be invalid, illegal or unenforceable for any reason, the remaining provisions shall continue in full force and effect and the invalid provision shall be reformed to the minimum extent necessary to make it valid and enforceable.

13. AMENDMENT

No amendment, modification, or waiver of any provision of this Waiver shall be effective unless in writing and signed by both parties.

14. WAIVER

The failure of any party to enforce any right or provision under this Waiver shall not constitute a waiver of such right or provision unless acknowledged and agreed to in writing by the party entitled to such right.

15. COUNTERPARTS; ELECTRONIC SIGNATURE

This Waiver may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one instrument. Signatures delivered by electronic means shall be treated as original signatures for all purposes.
