LEGAL PLATE RENEWAL FORM

This Legal Plate Renewal Agreement is entered into on by and between Registrant Name: (hereinafter "Registrant") and Renewal Agent Name: (hereinafter "Agent").

RECITALS

WHEREAS, Registrant is the legal owner or lawful registrant of the motor vehicle described below and desires to renew the vehicle's license plate and associated registration documentation in accordance with applicable law; and

WHEREAS, Agent is authorized to prepare, submit and execute renewal applications and related documents on behalf of the Registrant and to accept delivery of renewed plates and registration materials; and

WHEREAS, the parties wish to set forth the scope of the Agent's authority and the Registrant's certifications and acknowledgments concerning the renewal process.

NOW THEREFORE, in consideration of the mutual covenants set forth herein, the parties agree as follows:

2. AUTHORITY AND SCOPE OF SERVICES

2.1 Agent is authorized to prepare and submit renewal applications, sign applicable renewal documents on behalf of Registrant where permitted by law, pay required fees using funds provided under Section 3, and to collect and accept delivery of renewed license plates, registration cards and related materials. Agent's authority does not include the authority to alter the Registrant's ownership interest in the vehicle.

2.2 Agent shall perform the services in a diligent and timely manner consistent with customary professional standards applicable to vehicle registration services, and in accordance with applicable statutes and administrative rules.

3. FEES, PAYMENT AND AUTHORIZATION

3.1 Registrant shall pay Agent a service fee in the amount of in addition to any governmental fees, taxes, or surcharges required for renewal.

3.2 Payment shall be made by Registrant to Agent in the following manner:

4. REPRESENTATIONS, WARRANTIES AND DOCUMENTS

4.1 Registrant represents and warrants that Registrant is the lawful owner or authorized registrant of the vehicle described herein, that all information provided to Agent is true, complete and accurate, and that there are no undisclosed liens or encumbrances that would preclude renewal.

4.2 Registrant shall provide Agent with all necessary original or certified documentation required for renewal, including but not limited to proof of identity, title or registration documents, emissions or inspection certificates if required, and any other documentation reasonably requested by Agent.

5. INDEMNIFICATION

Registrant shall indemnify, defend and hold harmless Agent and its employees, agents and contractors from and against any and all claims, liabilities, losses, damages, costs and expenses, including reasonable attorneys' fees, arising out of Registrant's misrepresentations, failure to provide accurate documentation, or failure to disclose material facts reasonably required for the renewal process.

6. TERM, SUSPENSION AND TERMINATION

This Agreement shall commence on the date first written above and shall continue until the completion of the renewal services unless earlier terminated by either party for material breach after thirty (30) days' written notice to the other party. Agent may suspend services pending receipt of required documents or payment.

7. NOTICES

All notices, requests, demands and other communications required or permitted under this Agreement shall be in writing and shall be delivered to the addresses set forth below or to such other address as a party may specify in writing.

8. AMENDMENT; WAIVER; COUNTERPARTS

Any amendment or modification of this Agreement must be in writing and executed by both parties. No failure or delay by either party in exercising any right shall operate as a waiver. This Agreement may be executed in counterparts, each of which shall be deemed an original.

9. GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY

This Agreement shall be governed by and construed in accordance with the laws of the state in which the Registrant's vehicle is registered. This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior discussions and agreements. If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.

10. MISCELLANEOUS PROVISIONS

10.1 Confidentiality: Agent shall maintain the confidentiality of Registrant's personal information and use it solely for the purpose of performing renewal services, except as required by law.

10.2 Records: Agent shall retain copies of submitted renewal applications and related documents for a reasonable period and shall provide copies to Registrant upon request.

ACKNOWLEDGMENTS

Registrant acknowledges that Agent is not acting as an attorney and that the Agent's services are limited to administrative processing of the plate renewal. Registrant further acknowledges that certain circumstances (such as title issues, outstanding liens, or vehicle inspection requirements) may prevent renewal and that Agent's ability to obtain renewal is contingent upon resolution of those matters.
