What the Legal Power Designation Form Is

A Legal Power Designation Form is a written instrument that formally appoints an individual or entity to act on behalf of another person for specified legal, financial, or healthcare matters. It defines the scope of authority, any applicable limits or conditions, effective dates, and whether authority survives incapacity. The document may be durable (survives incapacity) or non-durable, and may require notarization or witnesses depending on the transaction type and state law. Properly completed forms create a clear record of delegated authority for third parties, financial institutions, and courts.
