What the Legal Professional Enquiry Form Is and when it’s used

The Legal Professional Enquiry Form is a structured intake document used to collect preliminary facts, contact details, conflict-check data, and matter-specific information from prospective clients or referring parties. It standardizes the intake process for law firms, in-house counsel, and legal service providers so staff can assess jurisdictional issues, identify potential conflicts, and scope engagement needs before drafting a retainer or undertaking substantive work. The form often includes sections for client identity, matter description, opposing parties, deadlines, and requested documents to ensure consistent triage and recordkeeping.

Why a standard enquiry form matters for legal matters

A consistent Legal Professional Enquiry Form reduces intake errors, speeds conflict checks, and creates a single authoritative record to start a client file.

Why a standard enquiry form matters for legal matters

Who prepares and completes the Legal Professional Enquiry Form

Intake teams, attorneys, paralegals, and authorized administrative staff typically complete or review the enquiry form to determine next steps.

  • Intake coordinators conducting conflict checks and opening new matters
  • Attorneys evaluating jurisdiction, statute of limitations, and case viability
  • Prospective clients or referrers supplying factual background and contact details

Some firms provide the form for prospective clients to complete online before an initial consultation; staff then validate entries and attach supporting documents.
