WHEREAS, both parties wish to record the scope, responsibilities, and acknowledgements attendant to Client's self-representation to avoid misunderstanding and to preserve the orderly administration of the matter.

NOW, THEREFORE

Client shall be solely responsible for preparing and filing all documents, attending all hearings, complying with applicable rules of procedure, and meeting all deadlines applicable to the matter. Client acknowledges that the Other Party may continue to be represented by counsel and that the rights and obligations of the parties shall be governed by applicable procedural and substantive law.

2. KNOWLEDGE, RISKS AND WAIVER

3. CONTACT INFORMATION AND SERVICE

Unless otherwise ordered by a tribunal, service of process, notices, correspondence, and documents to a party shall be effective if delivered to the mailing address or email address provided above. A party that changes its contact information must deliver written notice of the change to the other party in accordance with the Notices provision below.

4. CONFIDENTIALITY; PRIVILEGE

Client acknowledges that by proceeding without counsel certain privileges and protections ordinarily arising from attorney-client communications do not apply to communications with third parties. Client retains any privileges that exist by law; however, Client is responsible for preserving claimed privileges and must follow applicable rules and orders regarding confidentiality and disclosure.

5. FILINGS, DEADLINES AND SANCTIONS

Client acknowledges responsibility for calendaring and complying with all filing deadlines, hearing dates, and procedural requirements. Client understands that failure to comply may result in adverse rulings, dismissal, default, or other sanctions as permitted by law or court rule. Client shall not use the fact of self-representation as a basis for delay or to harass opposing parties.

6. REPRESENTATIONS AND CERTIFICATIONS

Client certifies under penalty of perjury that the information provided in this form is true and correct to the best of Client's knowledge. Client further certifies that Client is not under guardianship or other incapacity that would prevent meaningful participation in litigation, unless such status has been fully disclosed in writing to the Other Party and any tribunal with jurisdiction.

8. FEES AND COSTS

9. GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which the matter is pending, without regard to conflict-of-law principles.

10. ENTIRE AGREEMENT; SEVERABILITY; AMENDMENT

This Agreement constitutes the entire understanding between the parties with respect to the subject matter hereof. If any provision is found to be invalid or unenforceable, that provision shall be severed and the remaining provisions shall remain in full force and effect. No amendment to this Agreement is effective unless in writing and signed by both parties.

11. WAIVER; COUNTERPARTS

The failure of a party to enforce any right or remedy under this Agreement shall not operate as a waiver of such right or remedy. What the Legal Self-Representation Form Is and when it’s used

The Legal Self-Representation Form is a standardized declaration used by an individual who chooses to appear and act on their own behalf in a legal matter without an attorney (pro se). It identifies the filer, the case or petition, contact information, and the scope of the self-representation, and it may include acknowledgements about understanding court procedures and responsibilities. Courts and administrative agencies use this form to register a party’s intent to proceed without counsel, to set service addresses, and to ensure the filer understands procedural obligations and communications expectations.

Why this form matters for pro se litigants

Using a clear Legal Self-Representation Form records your status, contact details, and consent to receive court notices; it reduces procedural delays and helps the court manage pro se communications consistently.

Why this form matters for pro se litigants

Who commonly files a Legal Self-Representation Form

Typical filers include individuals, small business owners, and parties in administrative proceedings who choose to proceed without counsel.

  • Individual litigants asserting civil claims or defenses without hiring an attorney, often to reduce legal costs or when disputes are straightforward.
  • Small business owners or sole proprietors representing business interests in small claims or administrative hearings on a limited budget.
  • Respondents or petitioners in administrative matters (licensing, benefits, landlord-tenant) who will manage filings, service, and hearings themselves.

Courts accept pro se status but expect compliance with procedural rules; use the form to set clear expectations and service instructions.

Core elements included in a professional Legal Self-Representation Form

A well-constructed form collects identity, contact details, case identifiers, scope of representation, consent statements, and signature blocks so courts can rely on accurate service and communication data.

  • Mailing Address: Send a certified copy when required for proof of service.
  • Email Filing: Use official court email only if the court accepts email filings.

  • Recordkeeping: Tamper-evident audit trail

Use platforms that support ESIGN and UETA legal tests, preserve audit trails, and can export court-ready PDFs with signature certificates.

Common filing and service timing considerations

Timelines for filing and serving related pleadings vary by court; missing a deadline can affect case progression and remedies.

Initial Filing Deadline:

Check the summons or statute; deadlines vary by claim and jurisdiction.

Service Deadline:

Most civil actions require service within a fixed period after filing; confirm local rules.

Response Time:

Defendants often have 20–30 days to respond; verify the specific court rule.

Hearing Dates:

Calendar hearings promptly and note continuance procedures to avoid default.

Amendment Period:

Motions to amend pleadings are time-limited; file before applicable cutoffs.

Common mistakes to avoid when preparing the form

  • Leaving required fields blank or using inconsistent names that prevent the clerk from matching the form to your case.
  • Using informal abbreviations or P.O. boxes for service addresses where physical service is required.
  • Failing to sign, date, or complete witness/notary sections when the court mandates notarization or attestation.
  • Assuming e-mail service is accepted without confirming the court’s rules and obtaining required consents.

Case Dismissal: Failure to comply
Sanctions: Court fines or orders
Default Judgment: Missed deadlines
Service Rejection: Improper address
Privacy Exposure: Public docketing
