What the Legal Solicitor Information Form Is

The Legal Solicitor Information Form collects key identity, firm, matter, and authorization details for a solicitor acting on behalf of a client or party. Typical entries include the solicitor’s full legal name, bar number or license identifier, law firm name, contact information, the matter or case number, and a clear scope-of-authority statement. Essential Elements of a Professional Form

A well-constructed Legal Solicitor Information Form combines identity verification, clear authority language, contact details, and traceable signatures to serve legal and administrative needs.

Clear Identification

Full legal name, bar/license number and issuing jurisdiction appear prominently to allow rapid verification against public bar directories and internal vendor records.

Firm Details

Include registered law firm name, office address, and departmental contact to prevent misdirected service and to support billing and vendor management.

Scope of Authority

A concise authorization clause (e.g., 'limited to filing and receiving service') prevents disputes over what actions the solicitor may take on the client’s behalf.

Contact Points

Direct phone, secure email, and mailing address for official correspondence—avoid listing only general firm reception contacts for service-sensitive matters.

Verification Fields

Placeholders for bar verification links, dates checked, or attachments (bar record screenshot, engagement letter) to document credential checks.

Signature and Date

An explicit signature block with printed name, role, and MM/DD/YYYY date. For digital workflows include an audit trail to capture timestamp and IP.
