What the Legal Will Information Form Is

The Legal Will Information Form is a structured questionnaire used to collect the facts an attorney or probate court needs to prepare or admit a last will and testament. It gathers testator identity, family and beneficiary details, asset inventories, executor appointments, signatory blocks, and any special funeral or guardianship wishes. The form streamlines drafting, reduces information gaps before attorney review, and creates a clear record that supports probate administration and estate planning decisions.

Why a Completed Will Information Form Matters

A complete form ensures essential facts are captured accurately before drafting or probate. It reduces follow-up, helps identify conflicts, and supports a clearer executor transition while preserving the testator’s intent within applicable legal frameworks such as ESIGN and state probate laws.

Why a Completed Will Information Form Matters

Core parts of a professional Legal Will Information Form

A professional will form groups information logically so attorneys and courts can locate facts quickly and confirm formal requirements are met.

Full legal name, date of birth, social security number placeholder, current address, and government ID references to verify identity for probate and notarization.

Family and Heirs

Spouse, children, grandchildren, and predeceased relatives with full names, dates of birth, and relationship descriptions to determine intestacy contingencies and beneficiary designations.

Asset Inventory

Executors and Trustees

Primary and alternate executor or trustee names, contact information, powers granted, and whether bond or additional reporting is requested for fiduciary administration.

Specific Bequests

Clear descriptions of gifts to named beneficiaries, including conditions, monetary amounts, and contingent beneficiaries to prevent ambiguity during distribution.

Witness and Notary Instructions

Spaces for execution date, signature blocks, witness names and signatures, and instructions for a self-proving affidavit or remote online notarization if authorized by state law.
