What a Lender Information Sheet Is and when it’s used

A Lender Information Sheet is a standardized form that collects the details a lender needs to process credit, loan or financing requests. It typically records borrower identity, loan purpose, collateral details, account and tax identifiers, contact information, and authorization to obtain credit reports. Lenders use this sheet at application, pre-closing, or underwriting stages to confirm eligibility and complete regulatory checks such as identity verification and AML screening. The form is often attached to loan files and becomes part of the permanent borrower record for audit and compliance purposes.

Why the Lender Information Sheet matters to lenders and borrowers

A clear Lender Information Sheet speeds underwriting, reduces back-and-forth requests, and provides an auditable intake record.

Why the Lender Information Sheet matters to lenders and borrowers

Who typically completes or signs this form

The Lender Information Sheet is completed by parties with direct responsibility for loan intake and approval.

  • Loan officers and originators — collect borrower details and initial disclosures during application intake.
  • Underwriters and credit analysts — verify fields, supporting documents, and eligibility for approval decisions.
  • Borrowers or authorized representatives — provide identity, contact, income, and consent to credit checks.

Use the sheet across origination, underwriting, closing, and servicing functions to maintain a single source of truth.

Common preparation errors to avoid

  • Incomplete or mismatched names between ID and application causing identity verification failures and processing delays.
  • Missing or incorrect TIN entries that trigger backup withholding or require corrected tax reporting documentation.
  • Vague collateral descriptions that prevent accurate valuation or create underwriting exceptions and longer review times.
  • Unsigned or undated signature blocks that invalidate the form for closing or compliance purposes.

Consequences and compliance risks of incorrect information

Incorrect TIN: May trigger 24% backup withholding
Late reporting: Penalties under IRC §6721 for incorrect returns
I-9 errors: Fines for employment verification violations
Undetected fraud: Civil and criminal exposure risk
Funding delay: Loan close may be postponed
Data breach: Regulatory fines and remediation costs
