In consideration for offered to me by , I agree to release the Company and the Company's employees, directors, officers, and successors from all liabilities, claims, and causes of action, known or unknown, contingent or fixed, that I may claim as a result of my termination.

I agree not to file any lawsuit or other action to assert a claim.
