Core Sections That Make the Intake Form Actionable

Essential sections that make the LiftRight Client Intake Form actionable include client identity, service scope, logistics, billing, consents, and attachments for verification.

Client Identity

Collect full legal name, date of birth if required, government ID type and number only when necessary, and primary contact. Accurate identity fields reduce verification friction and support billing and tax compliance across jurisdictions.

Contact Details

Include mailing and service addresses, email, and phone numbers with preferred contact method. Use two-letter state abbreviations and verify ZIP codes; incorrect addresses can delay service or legal notices.

Service Scope

Describe requested services, estimated duration, materials or equipment needed, on-site access instructions, and any special handling requirements. Precise scope reduces change orders and clarifies liability for both parties.

Billing & Tax

Record billing name, billing address, payment terms, and taxpayer identification (SSN or EIN). Specify invoicing cadence and who is responsible for taxes or withholding to prevent payment disputes.

Consent & Authorization

Secure explicit consent for services, photo releases, and third-party access. For electronic execution, present ESIGN consumer disclosure and obtain affirmative consent before proceeding.

Supporting Attachments

Allow uploads for proof of identity, property access permits, insurance certificates, or site photos. Require file naming conventions and limit file sizes to keep intake consistent and audit-ready.

Security and Compliance Features to Note

Encryption: AES-256 at rest, TLS 1.2/1.3 in transit
Audit Trail: Timestamped actions, IP, signer audit logs
Access Controls: Role-based permissions and SSO
Compliance: ESIGN, UETA, SOC 2 Type II, ISO 27001
HIPAA: BAA available for protected health information

Common Preparation Errors to Avoid

  • Mismatched identity details between intake and ID slow verification, may prevent onboarding, and can trigger tax withholding or additional document requests.
  • Using vague service descriptions like 'as needed' creates pricing disputes, increases change orders, and complicates liability determinations between parties.
  • Failing to attach required supporting documents (IDs, permits, insurance) forces manual follow-up, delays scheduling, and undermines audit readiness.
  • Relying on weak signer authentication (email only) increases risk of unauthorized access and weakens enforceability for certain regulated transactions.

Penalties and Risks of Incorrect or Incomplete Intake

Incorrect TIN: Triggers 24% backup withholding
Missing Consent: May render electronic signature unenforceable
Late Filing: 1099 penalties $60–$330 per form
I-9 Errors: Civil fines $281–$2,789 per violation
