What the Load Delivery Requirements document covers

Load Delivery Requirements is a standardized document used by shippers, carriers, and receivers to specify conditions for physical delivery of freight. It outlines pickup and delivery addresses, appointment windows, required equipment, weight and dimension limits, packaging and loading instructions, hazardous materials disclosures, special handling needs, required documentation such as bills of lading or manifests, and proof-of-delivery procedures to reduce disputes and clarify financial responsibility for accessorial charges and delays.

Why a clear Load Delivery Requirements form matters

A clear Load Delivery Requirements form reduces handoff ambiguity, records agreed delivery conditions, and speeds dispute resolution. It helps stakeholders allocate liability for accessorials, improves billing accuracy, and creates a reproducible record for insurance and regulatory review.

Why a clear Load Delivery Requirements form matters

Which roles typically prepare and rely on this form

Primary users include shippers, motor carriers, freight brokers, and receiving agents who coordinate loading and delivery logistics.

  • Shippers: prepare load details, special handling, and required documentation for carriers.
  • Carriers: confirm equipment, appointment windows, and liability terms before acceptance.
  • Consignees/receivers: verify delivery access, unloading responsibility, and proof-of-delivery signing procedures.

Clear role alignment ensures each party knows responsibilities for documents, equipment, and proof at the time of delivery.

Essential sections to include in a professional form

Core sections of a professional Load Delivery Requirements form organize logistics, liability, timing, equipment, documentation, and proof-of-delivery instructions for clarity and enforceability.

Load Details

Describe commodity, gross weight, dimensions, pallet counts, and any hazardous materials. Include temperature control requirements and packaging specifications to ensure carrier readiness and compliance with DOT rules.

Pickup/Delivery

Provide full addresses, contact names, phone numbers, loading dock details, gate codes, expected appointment windows, and earliest and latest acceptable delivery times to avoid detention charges.

Equipment

Specify required tractor/trailer types, liftgate or pallet jack needs, maximum floor load, and any overweight permits or escort vehicle obligations to prevent refusal at delivery.

Documentation

List required paperwork such as bill of lading, manifest, customs documents, permits, and certifications; instruct which party provides each document and acceptable electronic formats for e-submission.

Accessorials

Define responsibility for detention, demurrage, reconsignment, storage, and special handling charges, including applicable rates, grace periods, and approval procedures for additional services.

Proof of Delivery

State required POD format (signed paper, scanned image, or electronic acknowledgment), acceptable signers, time-stamped photographs, and retention period for proof records.
