What the Location Information Questionnaire Captures

The Location Information Questionnaire is a structured form used to collect and centralize site-specific facts about a property, facility, or project location. Typical entries include full street and mailing addresses, parcel or legal descriptions, latitude/longitude coordinates, on-site contact names and phone numbers, access and security instructions, utility connections, zoning or land-use notes, and any known environmental hazards. Organizations use the questionnaire to support permitting, vendor bids, inspections, emergency response planning, and regulatory submissions, reducing repeated site visits and creating a single verifiable record for stakeholders.

Why a Standardized Location Questionnaire Matters

A consistent Location Information Questionnaire reduces ambiguity, speeds permitting and quoting, and centralizes access and hazard details. It improves coordination between field teams, contractors, and regulators while lowering the risk of site delays and rework caused by incomplete or inconsistent location data.

Why a Standardized Location Questionnaire Matters

Typical Users and How They Benefit

Organizations complete questionnaires to align operational, compliance, and field teams on site facts before mobilization.

  • Project managers and site supervisors collecting access, schedule, and equipment delivery details.
  • Environmental and compliance officers documenting hazards, permits, and regulatory contacts for filings.
  • Estimators and vendors needing precise coordinates, utility info, and service access notes.

A verified questionnaire becomes the single source of truth for scheduling, permits, and incident response across stakeholders.

Who Typically Completes and Signs the Form

Site Manager

Responsible for daily site operations and access control. Completes the questionnaire with gate codes, on-site contact names, operating hours, and precise physical address entries. Their accurate inputs determine scheduling, safety briefings, and permissions for contractors, reducing day-of confusion and unnecessary delays.

Compliance Officer

Oversees regulatory filings and permit accuracy. Uses questionnaire data to confirm zoning classifications, permit IDs, environmental hazards, and agency contacts. Precise responses support audits and legal reviews; inaccurate entries can delay approvals or require corrective filings with regulators.

Security and Compliance Checklist

Encryption in transit: TLS 1.2 and 1.3 in transit
Encryption at rest: AES-256 encryption for stored data
Certifications: SOC 2 Type II; ISO 27001; PCI DSS
Healthcare compliance: HIPAA compliant; BAA required
eSignature law: Supports ESIGN and UETA requirements
Accessibility: WCAG 2.0 Level AA support

Step-by-Step: Completing the Questionnaire

Follow these steps to fill out a Location Information Questionnaire accurately before permitting, inspection, or field mobilization.

  • 01
Collect identifiers: Gather parcel ID, full address, and GPS coordinates.
  • 02
Record access details: Include gates, hours, parking, and entry codes.
  • 03
Confirm utilities: Note available services and meter locations.
  • 04
Attach evidence: Add photos, scaled map, and permit copies.

Where Completed Questionnaires Move Next

This routing sequence shows typical handoffs from submission to verification, approval, and archival so teams know who reviews each field and when.

  • Submit: Sender uploads form and supporting files.
  • Verify: Reviewer cross-checks coordinates and documents.
  • Approve: Authorized signer validates and executes the record.
  • Archive: Store final record with audit trail and backup.

Essential Sections to Include in the Questionnaire

A professional Location Information Questionnaire contains structured sections that ensure completeness and legal clarity for operations, permitting, and audits.

Physical Location

Enter full street address, city, state, ZIP code, and the legal description if available. Use official formatting to match permitting systems and avoid correspondence or delivery errors that can delay work.

Coordinates & GIS

Supply latitude/longitude in decimal degrees and, when helpful, UTM or local grid references. Confirm coordinates on a trusted mapping tool so crews and inspectors reach the correct parcel.

Access & Security

Detail gate codes, restricted hours, secured areas, parking, and required PPE. Clear access notes reduce delays, prevent denied entry, and improve on-site safety for contractors and inspectors.

Utilities & Services

List active utility providers, meter IDs, service types, and capacity notes. Flag temporary service needs and easements that affect routing of equipment or scheduling of work.

Environmental & Hazards

Document flood zones, wetlands, asbestos, underground utilities, and other hazards. Note required permits or mitigation steps so reviewers can assess compliance impacts immediately.

Attachments & Evidence

Attach timestamped photos, survey excerpts, permit copies, and scaled maps. Stamped plans and dated images strengthen the record for audits and dispute resolution.

Practical Tips for Accurate Completion

Use these best practices to reduce errors, improve usability, and ensure the questionnaire supports downstream workflows and regulatory needs.

Standardize address formats
Always use USPS address formatting and avoid abbreviating legal locality names. Consistent addresses prevent mismatches with permitting databases and reduce returned mail or misrouted inspections.
Verify coordinates on map
Confirm GPS coordinates against a reliable mapping source and cross-check parcel boundaries. Even small coordinate errors can place crews on an adjacent parcel and cause costly delays.
Use clear contact details
Provide full names, direct phone numbers, email, and a secondary contact with reachable hours. Accurate contact data prevents missed visits and speeds issue resolution on site.
Keep records versioned
Include revision dates and editor initials when updating questionnaires. Version control clarifies which dataset governs permits, work orders, or emergency response actions.

Typical Timelines and Processing Expectations

Processing time varies by agency and internal workflow; these common timing expectations help plan reviews, permits, and mobilization.

Initial submission:

Submit at least 10 business days before planned mobilization.

Internal review:

Allow 3–5 business days for verification.

Permit filing:

Agency review windows commonly run 2–6 weeks depending on scope.

RON notarization:

If used, remote notarization session records typically retained 5–10 years.

Record retention:

Archive final document per compliance schedules and corporate policy.

Penalties and Risks from Inaccurate Location Data

Incorrect Tax IDs: Triggers 24% backup withholding
Late returns: $60–$330 per information return
Intentional disregard: $660+ per form, no cap (IRC §6721)
I-9 paperwork: $281–$2,789 per violation (8 CFR §274a.2)
Notarization errors: May void document or delay filing
Mismatched names: Can invalidate signatures or require reexecution

Common Mistakes to Avoid

  • Using informal address formats that conflict with permitting or GPS mapping systems.
  • Failing to verify coordinates, which relocates crews to the wrong parcel.
  • Omitting access restrictions or gate codes that prevent site entry.
  • Not versioning updates, causing teams to rely on outdated location data.

Practical Use Cases

These examples show how standardized location questionnaires reduce rework and improve field outcomes in real projects.

Development Site

A regional developer uses the questionnaire to consolidate parcel, access, and utility facts before bidding site work.

  • This reduced ambiguity and clarified scope for bidders.
  • With consistent location data subcontractors received accurate scopes and bids, avoiding change orders and expediting permitting and inspections across multiple parcels simultaneously.

Healthcare Clinic

A community health center records facility addresses, entry points, and emergency contacts to streamline outreach and mobile services.

  • This supports HIPAA-aware routing and scheduling.
  • Accurate site data reduced missed appointments, ensured correct delivery of medical supplies, improved patient routing, and provided compliance-ready records for audits and incident response planning.
