What the Marketing Advertising Request Form Is

The Marketing Advertising Request Form is a standardized document used by organizations to capture campaign requirements, creative assets, target audiences, budget limits, timelines, approval authorities, and delivery instructions for paid and organic marketing activities. It centralizes project details so marketing, creative, media, legal, and procurement teams can evaluate scope, confirm resource needs, and schedule production. Completed requests typically include attachments (briefs, brand guidelines, media specs) and specified dates for drafts, revisions, and final launches, enabling clearer accountability and a documented audit trail for internal approvals and external vendor work.

Why using a standardized request form matters

Using a Marketing Advertising Request Form standardizes intake, reduces miscommunication, and speeds review cycles by ensuring all stakeholders receive the same brief, file attachments, budget constraints, and approval paths. It helps prioritize requests and creates a clear record for compliance and post-campaign analysis.

Why using a standardized request form matters

Who completes and relies on this form

Marketing, creative, and media teams complete the Marketing Advertising Request Form to initiate campaigns and request agency or vendor support.

  • Marketing managers or campaign owners specifying objectives, budgets, and target dates.
  • Creative leads providing assets, format specs, and brand compliance notes for production.
  • Procurement or legal reviewers checking vendor terms, licensing, and regulatory compliance.

Approvers and finance validate budgets, while external agencies receive completed forms to begin scheduling and execution.

Typical requestor and agency roles

Marketing Manager

A marketing manager who authorizes campaign requests, sets budget limits, chooses target segments, and tracks approvals. They typically attach briefs, define KPIs, and delegate creative requests. Their submission starts internal reviews and signals resource allocation and scheduling needs across departments.

Agency Account Director

An agency account director receives the completed request to scope media buys, confirm creative format compliance, and plan timelines. They review provided assets, identify gaps, and submit production estimates. Their role turns internal requirements into vendor-level schedules and cost proposals.

Core sections to include on a professional request form

Include consistent sections to capture scope, assets, budget, schedule, approvals, and measurement criteria so teams can estimate effort and confirm compliance before work begins.

Project Scope

Describe campaign objectives, target KPIs, channels, deliverable formats, and any mandatory creative elements. Clear scope prevents scope creep and supports accurate vendor bids and internal resource planning.

Assets & Specs

List every file required: sizes for images, video codecs and durations, font files, and editable source files. Specify naming conventions and preferred color profiles to reduce back-and-forth with production teams.

Budget & Fees

State total budget, media allocation, production fees, contingency amounts, and invoicing terms. Clarify if taxes, agency commissions, or third-party fees are excluded or included in the stated total.

Timeline

Provide milestone dates for first drafts, revisions, final assets, trafficking, and launch. Note blackout dates, campaign flighting windows, and dependency constraints that affect scheduling.

Approvals

Include approver names, roles, contact details, and expected decision timelines. Use explicit approval type (email, eSign, or written memo) and note any delegated signing authority.

Measurement

Define success metrics, reporting cadence, tracking pixels, UTM naming, and attribution model. Attach reporting templates and baseline KPIs to avoid post-launch disputes about performance.

Step-by-step: filling out the request

Follow these steps to submit a clear, approvable Marketing Advertising Request Form and avoid common intake delays.

  • Shared Link: Public or tokenized links for guest signers.
  • Integrations: Connect to CRM, collaboration, cloud storage.

Typical timelines, deadlines, and review windows

Track submission, review, deliverable, and launch dates to align creative development, vendor procurement, and paid media activation timelines.

Submission Lead Time:

Allow 10–15 business days for creative, media planning, and approvals.

Creative Turnaround:

Standard design revisions take 3–7 business days.

Legal Review:

Legal reviews typically require 2–5 business days.

Procurement Processing:

PO issuance can take 3–10 business days.

Launch Window:

Reserve media slots at least seven days before start.

Common mistakes that delay campaigns

  • Incomplete asset attachments such as missing image sizes, fonts, or video masters cause iterative reviews and production hold-ups that extend campaign timelines.
  • Ambiguous targeting instructions (broad age ranges, undefined geographies, or conflicting audience segments) lead to wasted media spend and poor performance measurement.
  • No approval path or missing approver contact information stalls signoff; lack of deadlines creates scope creep and untracked scope changes.
  • Incorrect budget classification or omitted tax details can cause reallocation delays and errors in vendor invoices or internal cost reporting.

Risks and downstream consequences of incorrect forms

Budget Overruns: Unapproved spend, cost recovery issues.
Schedule Delays: Missed launch windows and lost impressions.
Regulatory Noncompliance: HIPAA or FTC disclosure failures.
Contract Rejection: Vendors may refuse incorrect requests.
Tax Reporting Issues: Incorrect invoices affect 1099 filings.
Payment Delays: Finance holds until approvals complete.

Practical examples of structured request intake

Real-world examples show how structured requests reduce friction and speed campaigns in varied organizational contexts.

Martin Properties — Tim Martin

Martin Properties standardized online request intake and executed documents entirely digitally to manage property marketing at scale.

  • Reduced turnaround and improved compliance.
  • The firm completed approvals remotely, kept secure audit trails, and shortened time-to-launch for listings and paid media, improving operational throughput without adding headcount while reducing paper storage and simplifying vendor billing reconciliation.

Xerox — Kodi-Marie Evans

Xerox integrated request forms with NetSuite to route approvals and ensure the right signature flows for marketing contracts.

  • Enabled flexible formats and templates.
  • Integration reduced manual entry, allowed programmatic document generation, and ensured signatures matched accounting records, simplifying reconciliation and accelerating vendor payments across global teams while preserving audit trails and compliance artifacts for internal controls and external audits.

Best practices to improve accuracy and speed

Follow these practical tips to reduce errors, accelerate approvals, and ensure the Marketing Advertising Request Form consistently supports production and compliance needs.

Use standard naming and file conventions
Adopt consistent file names, size conventions, and color profiles. Provide editable source files when required and compress large assets per spec. Clear conventions reduce back-and-forth, speed production handoffs, and lower the chance of versioning errors during launch windows.
Define approval SLAs and escalation paths
Set explicit decision timelines for each approver and define an escalation path for missed approvals. Communicate SLAs to vendors and internal teams; use automated reminders and conditional routing to keep campaigns on schedule and reduce unplanned delays.
Validate assets before submission
Run preflight checks on images, fonts, and videos to ensure they meet format, resolution, and codec requirements. Flag any third-party licensing needs up front to prevent legal holds during review and production.
Keep a single source of truth
Store the approved brief, final assets, and signed approvals in a centralized repository. Link requests to project IDs and invoices so finance and audit teams can reconcile spend without manual searches or duplicate records.
