What the Marketing Campaign Request Form Is and Who Uses It

A Marketing Campaign Request Form is a standardized internal document used to collect all information required to plan, approve, build, and launch a marketing campaign. It centralizes objectives, target audiences, creative assets, budgets, tracking requirements, and approval routing so project managers, creative teams, legal, and finance have a single source of truth for execution and reporting.

Why a Standardized Request Form Matters

Using a single form reduces ambiguity, speeds approvals, and improves handoffs between marketing, design, legal, and procurement, helping ensure campaigns meet brand, compliance, and budget requirements.

Why a Standardized Request Form Matters

Typical Users and Stakeholders

This form is completed by the campaign owner and routed to key stakeholders for review and approval.

  • Marketing Managers and Campaign Owners — Submit campaign goals, channels, target audience, and budget details for approval.
  • Creative and Production Teams — Receive asset requirements, formats, sizes, and deadlines to produce ads and landing pages.
  • Legal, Compliance, and Data Privacy Teams — Review messaging, disclosures, and data use for regulatory compliance and contractual obligations.

Final signers typically include the marketing director, budget owner, and any compliance or legal approvers required by company policy.

Core Sections a Professional Form Should Include

A complete request form groups the campaign brief, audience, assets, tracking, budget, and approval workflow so reviewers can evaluate scope and risks quickly.

Campaign Brief

Summarize objectives, success metrics, primary message, and campaign type (email, paid social, display, search, event).

Targeting

Define target segments, geographies, exclusions, and any audience suppression lists to avoid compliance issues.

Creative Assets

List required ad sizes, copy variants, landing pages, and who is responsible for delivery and deadlines.

Tracking & Measurement

Identify UTM parameters, pixels, conversion events, attribution model, and reporting cadence.

Budget & Billing

Provide total spend, media allocation, cost center, PO number if required, and invoicing contact information.

Approvals Workflow

Specify required approvers, sign-off order, legal reviews, and the expected timeline to launch.

Required Information and Data Points

Campaign Name: Unique identifier
Owner: Requester name
Start/End Dates: MM/DD/YYYY
Budget: Total spend
Target Audience: Segment details

How to Configure the Online Request Workflow

Conditional Logic Show additional inputs when channel equals 'paid media' or budget exceeds threshold.
Approval Sequence Set role-based routing and enforce signer order where needed.
Notifications Enable email and in-app reminders for pending approvals.

Where the Completed Form Goes and What Happens Next

After submission the form routes automatically, notifies stakeholders, and logs each action for audit and reporting.

  • Routing: Sent to approvers in configured order.
  • Notifications: Email and platform alerts notify reviewers.
  • Approvals: Signers approve or request changes inline.
  • Archival: Final form stored in document repository.

Distribution and eSubmission Options

  • Integrated Systems: Embed form in CRM or marketing hub via integration.
  • In-Person Kiosk: Provide device-based signing for on-site approvals.

Use integrations with common platforms such as CRM and cloud storage to centralize approvals and maintain a single source of record.

Typical Timelines and Internal Deadlines

Set internal deadlines to allow time for creative production, legal review, vendor onboarding, and platform setup before campaign launch.

Submission Lead Time:

Submit requests at least 10 business days before planned launch.

Creative Delivery:

Creative assets required 7 business days before launch for QA.

Legal Review Window:

Allow 3–5 business days for compliance and legal review.

Media Setup:

Ad account and pixel configuration require 2–4 business days.

Reporting Turnaround:

Initial performance report delivered within 7 calendar days post-launch.

Common Mistakes to Avoid

  • Incomplete targeting details causing mismatched audiences and wasted media spend when campaigns go live.
  • Late asset delivery that compresses QA time and increases approvals needing rework after launch.
  • Vague measurement specs that prevent reliable attribution and make campaign performance hard to validate.
  • Missing approver details that delay routing and can halt the campaign close to launch date.

Risks and Regulatory Consequences to Watch For

Data Privacy Violations: Failure to honor opt-outs can trigger regulatory investigations and fines.
CAN-SPAM Noncompliance: Incorrect email consent handling may lead to complaints and penalties.
Misleading Claims: Unvetted messaging can lead to legal exposure and consumer restitution.
Third-Party Breaches: Vendor data incidents can create contractual liability for the advertiser.
Brand Safety Issues: Poor placement controls may damage reputation and require costly remediation.
Contractual Breach: Failure to meet advertiser or partner terms risks indemnities and lost revenue.
