MARTIAL ARTS TRAINING AGREEMENT

WHEREAS

WHEREAS, School/Instructor Name: operates a martial arts training program and provides instruction, coaching, and related services; and

WHEREAS, Student Name: (if a minor, Guardian Name: ) seeks to enroll and participate in the School's training program under the terms set forth in this Agreement; and

NOW, THEREFORE, in consideration of the mutual covenants and promises contained herein, the parties agree as follows.

PARTIES AND CONTACT INFORMATION

SCOPE OF TRAINING

The School agrees to provide martial arts instruction, coaching, sparring supervision, grading, and related services as described below. Instruction shall be conducted by qualified instructors and shall include technical training, physical conditioning, and safety supervision appropriate to the student's level.

PAYMENT TERMS

Student agrees to pay tuition, fees, and other charges as set forth below. Fees cover instruction, facility use, and grading administration unless otherwise specified.

All payments not received when due shall accrue the specified late payment fee and may be subject to suspension of training until the account is current. Returned payment items will incur an administrative charge.

TERM AND TERMINATION

This Agreement commences on Start Date: and continues until End Date: , unless earlier terminated in accordance with this Agreement.

Either party may terminate this Agreement for material breach that is not cured within the notice period. The School may immediately suspend or terminate training where the Student's conduct or health creates a risk to others or to the Student's own safety.

CONFIDENTIALITY

The parties acknowledge that in the course of training, certain information may be confidential, including proprietary training methods, curricula, and business information. The Student and Guardian agree not to disclose, reproduce, or use the School's confidential materials for any commercial purpose without prior written consent. This obligation survives termination of this Agreement for a period of two (2) years.

ASSUMPTION OF RISK, WAIVER AND MEDICAL DISCLOSURE

Martial arts training involves physical activity, contact, and risk of injury. The Student (or Guardian on behalf of a minor) acknowledges the inherent risks and voluntarily assumes all risks associated with participation. The Student releases and holds harmless the School, its instructors, agents, and employees from claims for bodily injury, property damage, or death arising out of participation, except to the extent caused by the School's gross negligence or willful misconduct.

In the event of a medical emergency, the School is authorized to obtain medical treatment for the Student and to take such action as reasonably necessary for the Student's health and safety. The Student/Guardian agrees to be responsible for all medical expenses incurred.

CONDUCT, ATTENDANCE AND EQUIPMENT

The Student shall comply with the School's rules of conduct, attendance, and safety. Repeated misconduct, failure to attend scheduled sessions without notice, or refusal to follow safety directions may result in suspension or termination without refund. The Student is responsible for providing appropriate personal protective equipment where required.

CANCELLATION AND REFUND POLICY

Refunds for prepaid tuition are provided only as set forth in the School's published policy. If the Student withdraws prior to the commencement of training, a refund less any nonrefundable registration fee may be issued. Once training commences, refunds are discretionary and may be prorated at the School's standard rate less administrative fees.

GOVERNING LAW; DISPUTE RESOLUTION

This Agreement shall be governed by and construed in accordance with the laws of the State of , without regard to its conflict of law principles. The parties agree that any dispute arising out of this Agreement will be resolved by good faith negotiation; if unresolved, the parties may pursue adjudication in a court of competent jurisdiction in that State.

ENTIRE AGREEMENT

This Agreement constitutes the entire understanding between the parties concerning the subject matter herein and supersedes all prior agreements, representations, and understandings, whether written or oral. Any amendment must be in writing and signed by both parties.

If any provision of this Agreement is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.
