What the Matrimonial Question Form Is and how it’s used

The Matrimonial Question Form is a structured questionnaire used in family law matters to collect factual detail about parties, marriage history, children, financial accounts, property, and other case-relevant facts. Courts, mediators, and attorneys use it during intake and discovery to create a consistent record and reduce repetitive questioning. Responses are typically entered into case files and may be converted into affidavits, pleadings, or exhibits. When handled electronically, follow applicable e-signature rules and retention practices to preserve admissibility and chain-of-custody information.

Why a standardized questionnaire improves family law workflows

A Matrimonial Question Form standardizes information collection, reduces omissions, and speeds case preparation. It provides counsel and the court with consistent, comparable data across parties, which simplifies review, evidentiary planning, and settlement analysis while preserving a record suitable for disclosure and filing.
