What the Media Interview Form Is and when it’s used

A Media Interview Form is a structured consent and information document used to schedule, document, and obtain permissions for interviews with journalists, producers, or content creators. It records interviewee contact details, affiliation, topics, time and place, recording and redistribution permissions, embargo terms, and any release language needed for broadcast or publication. Organizations use it to manage legal risks, ensure clarity on usage rights, and create an auditable record of consent before recording or publishing interview content.

Why a clear Media Interview Form matters

Using a standard Media Interview Form reduces ambiguity about permissions, protects against unauthorized use or distribution, documents consent for recording and quotes, and supports compliance with regulatory rules where sensitive information is involved.

Why a clear Media Interview Form matters

Who typically completes and signs this form

Media relations teams, communications officers, interview subjects, and legal or compliance staff commonly complete and approve this form before an interview.

  • Public relations professionals and spokespeople who arrange interviews and manage media logistics.
  • Individual interview subjects or their legal representatives who grant consent and confirm factual details.
  • Legal, compliance, or records staff who review release language and retention requirements.

Each signer should confirm identity, role, and consent explicitly to create a reliable audit trail for future reference.

Common mistakes to avoid when preparing interview forms

  • Using vague permission language that permits broader use than intended, which can lead to disputes over clips or syndication.
  • Failing to obtain guardian or minor consent when interviewing underage subjects, exposing the organization to legal risk and reputational harm.
  • Neglecting to record or retain the audit trail for electronic signatures, making it difficult to prove consent in a dispute.
  • Missing embargo terms or incorrectly recording them, which can cause premature publication and breach of agreements.

Risks and consequences of incorrect or missing consent

Unauthorized Use: Legal claims
Privacy Breach: HIPAA exposure
Defamation Risk: Civil liability
Embargo Violation: Contract breach
Invalid Release: unenforceable
Recordkeeping Fail: Evidence lost
