What the Meeting Referral Form Is and When to Use It

A Meeting Referral Form is a structured document used to nominate or refer an individual or party for a scheduled meeting, introduction, or internal handoff. It captures who is referring, who is being referred, the purpose of the meeting, preferred times, and necessary context so scheduling and preparation are efficient and auditable. Organizations use the form to centralize requests, track outcomes, and ensure appropriate stakeholders and resources are assigned before the meeting occurs.

Why a Standardized Meeting Referral Form Matters

A consistent referral form reduces back-and-forth, clarifies meeting objectives, and creates an auditable trail for follow-up and accountability.

Why a Standardized Meeting Referral Form Matters

Who Typically Completes and Receives This Form

Several roles commonly create and act on Meeting Referral Forms depending on organizational structure and use case.

  • Sales and business development teams: submit prospect introductions and handoffs to account executives or product specialists.
  • Human resources and recruiting: refer candidates or internal colleagues for interviews and panel meetings with scheduling details.
  • Patient intake or care coordinators (healthcare): refer patients to specialists with clinical context and preferred appointment windows.

Matching the form to the user role improves data quality and ensures the referred meeting proceeds with required context and participants.
