MEMBERSHIP MEMBER CONSENT FORM

RECITALS

The Organization will provide to the Member the services, access and benefits described below. The Member agrees that the scope set forth here defines the Member's entitled benefits and any exclusions or limitations.

PAYMENT TERMS

Membership Fee (one-time or recurring) shall be as set forth below. The Member authorizes the Organization to charge the payment method provided in accordance with the schedule and acknowledges responsibility for timely payment.

TERM AND TERMINATION

This Agreement commences on Start Date: and continues until End Date: , unless earlier terminated as provided below.

Either party may terminate this Agreement upon written notice delivered at least Notice Period (days): days prior to the intended termination date. Organization may suspend membership privileges for non-payment or material breach pending cure where applicable.

CONFIDENTIALITY

Member and Organization agree that Confidential Information disclosed in connection with this membership, including proprietary materials, membership lists, business practices and non-public pricing, shall be held in strict confidence and used solely for the purposes of performing obligations under this Agreement. Confidential Information does not include information that is or becomes public through no fault of the receiving party, or is lawfully received from a third party without obligation of confidentiality.

The receiving party shall use at least reasonable care to protect Confidential Information, which shall not be disclosed except to employees, contractors or agents with a need to know who are bound by comparable confidentiality obligations.

MEMBER CONSENTS & AUTHORIZATIONS

I consent to the Organization's use of my image and likeness for promotional materials and internal records in accordance with the Organization's policies.

I consent to the collection, storage and processing of my personal data for purposes of membership administration, billing and communication.

I authorize the Organization to perform such background or credit checks as are reasonably necessary to determine eligibility for certain membership levels, where applicable.

LIABILITY AND RELEASE

The Member acknowledges that participation in certain activities may involve inherent risks. To the fullest extent permitted by law, the Member releases the Organization, its officers, employees and agents from claims for personal injury, property damage or loss arising out of membership activities, except to the extent caused by the Organization's gross negligence or willful misconduct.

EMERGENCY CONTACT

GOVERNING LAW; ENTIRE AGREEMENT

This Agreement shall be governed by and construed in accordance with the laws of the State or jurisdiction identified by the Organization below. The parties consent to the exclusive jurisdiction of the courts located in that jurisdiction for any disputes arising under this Agreement.

This Agreement constitutes the entire agreement between the parties with respect to the subject matter herein and supersedes all prior and contemporaneous understandings, representations and agreements, whether written or oral. Any amendment must be in writing and signed by both parties.
