What the Mental Health Evaluation Confirmation Is

A Mental Health Evaluation Confirmation documents that a licensed clinician has assessed an individual and recorded findings relevant to care, treatment planning, or third-party requirements. It typically states the evaluator's credentials, the evaluation date, diagnostic impressions, functional status, and any recommended follow-up or restrictions. The confirmation may be requested by employers, schools, benefits administrators, legal parties, or other clinicians. Depending on purpose, it can support accommodations, leave requests, clinical referrals, or medico-legal assessments while protecting patient privacy and meeting recordkeeping requirements.

Why a Clear Confirmation Matters (Purpose and Legal Basis)

A concise Mental Health Evaluation Confirmation creates an auditable record of clinical findings and recommendations, clarifies the evaluator's role, and supports downstream decisions such as accommodations, benefits, or clinical referrals. When exchanged electronically, it must meet e-signature legal standards under the ESIGN Act (15 U.S.C. ch. 96) and applicable state law (UETA or a state's ESRA) to ensure enforceability.

Why a Clear Confirmation Matters (Purpose and Legal Basis)

Who Typically Prepares or Receives This Confirmation

Tailor the form fields to the recipient’s needs while keeping the clinician’s statement factual, dated, and signed to maintain credibility.

  • Clinicians and evaluators in outpatient, inpatient, and forensic settings who document diagnostic impressions and recommendations.
  • Human resources, disability services, or benefits administrators requesting documentation to evaluate leave or accommodations.
  • Legal representatives, courts, or insurance examiners who need an independent clinical statement for proceedings or coverage decisions.
