What the Migration Data Form Is and when it’s used

A Migration Data Form is a structured intake document used to record metadata, scope, and permissions before moving records between systems. It captures source system identifiers, data owners, record types, required transformations, retention rules, and authorization signatures so technical teams and stakeholders share a single source of truth. The form reduces ambiguity during extraction, mapping, and validation steps while documenting consent where personal or protected data is involved. Many organizations use the form to support audit trails, legal compliance, and rollback planning during large or phased migrations.

Why a Migration Data Form matters for reliable transfers

A clear Migration Data Form reduces data-loss risk, documents owner authorization, and creates an auditable record for compliance. It guides mapping, validation, and acceptance testing while making responsibilities explicit across teams and vendors.

Why a Migration Data Form matters for reliable transfers

Typical teams and roles that complete a Migration Data Form

Project and data teams complete the form to align technical scope and approvals before migration begins.

  • Project Managers coordinating scope, timelines, and vendor handoffs.
  • Data Owners responsible for data classification and access authorizations.
  • Legal/Compliance reviewers verifying PII/PHI handling and retention obligations.

The form also serves legal, compliance, and business contacts as a single source for permissions, retention, and acceptance criteria.

Stepwise process to complete and approve the form

Follow the sequence below to prepare the form, obtain approvals, and baseline migration artifacts before execution.

  • 01
Prepare: Gather inventories, sample exports, and owner contact details.
  • 02
Map: Define field-to-field mappings and transformation rules.
  • 03
Validate: Run sample exports and reconciliation tests with stakeholders.
  • 04
Approve: Obtain authorized signatures and record the acceptance criteria.

Configure an online form workflow for efficient processing

Set up authentication, conditional logic, and notifications so the form routes automatically to each reviewer.

Field Configuration
Authentication Email plus optional SMS code or SSO for stronger signer identity.
Conditional Fields Show data-specific sections only for selected source systems or data types.
File Types Allow CSV, JSON, and PDF attachments for sample exports.
Notifications Auto-notify owners and approvers when a form reaches their queue.

Typical routing and submission destinations

A completed Migration Data Form commonly routes to technical, business, and records teams and stores attachments in project repositories.

  • Project Repo: Attach signed form PDF and sample exports to project storage.
  • Compliance: Copy compliance reviewers for retention and policy checks.
  • Technical Lead: Send mapping and transformation artifacts to developers.
  • Audit Trail: Preserve signed form and history for future audits.

Platform and format requirements for electronic completion

Use a platform that supports PDF, DOCX, CSV attachments, conditional logic, and audit trails.

  • File Formats: PDF, DOCX, CSV, JSON supported for uploads.
  • Integrations: Connectors to cloud storage and project systems recommended.
  • Security: TLS in transit and AES-256 at rest required.

Core sections every professional Migration Data Form should include

A complete form addresses scope, permissions, technical mapping, validation, schedule, and the legal authorization needed to proceed.

Scope

Define exact systems, datasets, and exclusions. Clear scope prevents scope creep and avoids accidental transfers of unrelated or restricted records.

Data Inventory

Record data volumes, table names, and sensitivity classifications so capacity planning and masking requirements can be applied consistently.

Field Mapping

List source-to-target mappings and transformation logic to guide developers and reduce interpretation errors during ETL.

Validation Plan

Include reconciliation steps, sample queries, and acceptance thresholds to verify migrated data meets business requirements.

Schedule & Backups

Specify maintenance windows, backup procedures, and rollback plans to limit application downtime and preserve recoverability.

Authorization & Audit

Capture signed approvals, version history, and an audit trail to demonstrate consent and provide evidence for compliance reviews.

How to save and what supporting documents to attach

Store the completed form with export samples, mapping files, and approval records to create a durable migration record set.

Export Formats

Save signed forms as searchable PDF/A for long-term preservation; keep sample data in CSV or JSON for validation.

Signed Package

Include the signed form, certificate of completion, and any evidence of signer authentication in one package for audits.

Mapping Files

Attach mapping spreadsheets and transformation scripts so future teams can trace decisions and reproduce results.

Change Log

Record version history, reviewer comments, and approval timestamps to document who approved each change and when.

Real-world examples of Migration Data Forms in use

Organizations across industries use standardized forms to reduce errors and accelerate migration approvals while keeping compliance obligations visible.

Martin Properties

Martin Properties created a standardized intake form to record tenant and lease metadata before transfers.

  • Reduced turnaround by removing paper steps and clarifying owners.
  • "I can process and execute all of these documents online with 100% compliance and built-in security," said Tim Martin, describing faster closures and fewer manual reconciliations.

Xerox (NetSuite Operations)

Xerox used a form to collect source identifiers and authorization for ERP-to-ERP migration.

  • Ensured correct NetSuite mappings and fewer data mismatches.
  • "airSlate SignNow provides us with the flexibility needed to get the right signatures on the right documents," said Kodi-Marie Evans, emphasizing streamlined approvals and integration benefits.
