Assumption of Risk, Release, and Indemnity

In consideration of the minor's participation, Parent or Guardian hereby acknowledges and agrees that participation may involve inherent risks, including but not limited to physical injury, illness, or property damage. Parent or Guardian voluntarily assumes all such risks on behalf of the minor.

Parent or Guardian, on behalf of the minor, releases, waives, and forever discharges Organization, its officers, directors, employees, volunteers, agents, and representatives (collectively, Released Parties) from any and all liabilities, claims, demands, causes of action, or suits (including attorney's fees and costs) arising out of or in any way connected with the minor's participation, except to the extent caused by the Released Parties' gross negligence or willful misconduct.

Parent or Guardian further agrees to defend, indemnify, and hold harmless the Released Parties from any third-party claims arising from the minor's acts or omissions during participation.

Medical Authorization

In the event of illness or injury, Parent or Guardian authorizes Organization to obtain emergency medical treatment for the minor and to arrange transport to an appropriate medical facility. Parent or Guardian agrees to be responsible for all costs related to such treatment.

Confidentiality

The parties shall maintain as confidential all non-public information relating to one another and to the minor's health and any medical records obtained in connection with this agreement, except as required by law or necessary for emergency medical care. Confidential information shall not be disclosed without the prior written consent of the disclosing party.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the state specified by Organization: without regard to conflict of law principles.

Entire Agreement

This Agreement constitutes the entire agreement between the parties concerning the minor's participation and supersedes all prior and contemporaneous agreements, understandings, and representations, whether written or oral. Any amendment must be in writing and signed by both parties.

Miscellaneous Provisions

If any provision of this Agreement is held invalid or unenforceable, the remainder of the Agreement shall remain in full force and effect. The parties agree that a court may reform this Agreement to effectuate the parties' intent where possible.
