What the Miscellaneous COI & CAF Template Is

The Miscellaneous COI & CAF Template is a standardized form bundle combining a Certificate of Insurance (COI) summary and a related Contact/Authorization Form (CAF) used to document insurance coverage and authorize access or activity for contractors, vendors, or site visitors. It collects insurer and policy details, named insured, coverage types and limits, project or site identifiers, and contact and authorization data needed by owners, property managers, or contracting parties. The template is intended to create a consistent record that supports risk assessment, compliance review, and contract administration.

Why a Consistent COI & CAF Template Matters

A single, well-structured template reduces review time, ensures required insurance elements appear consistently, and preserves an audit-ready record for compliance and dispute resolution. It also clarifies who is authorized on site and links coverage data to specific projects or contract periods.

Why a Consistent COI & CAF Template Matters

Who Typically Completes and Reviews This Template

Organizations use the Miscellaneous COI & CAF Template to centralize risk and access information before permitting work or site access.

  • Property managers and owners who require proof of insurance and contractor authorization prior to site access
  • Procurement and contracting teams who need standardized insurance attestations tied to purchase orders or scopes
  • Facilities, safety, and risk managers who verify coverage limits, additional insured endorsements, and certificate holder status

Having defined reviewers and a clear submission process reduces delays and minimizes back-and-forth over missing certificate elements.

Digital Distribution and eSubmission Considerations
