What the Miscellaneous EFT Addendum Is and When It Applies

A Miscellaneous EFT Addendum is a contract attachment that documents authorization and operational details for electronic funds transfers related to an underlying agreement. It typically specifies payer and payee banking details, permitted transaction types, frequency, timing, return and error handling, liability allocation, and reconciliation procedures. Organizations use this addendum to standardize electronic payments, reduce paper check processing, and record consent to ACH, wire, or other EFT channels. Properly completed, it creates a clear, auditable record of payment instructions and the parties' agreement about electronic transfer mechanics and responsibilities.

Why Parties Use a Miscellaneous EFT Addendum

The addendum clarifies payment flow, assigns liability for returns and reversals, and captures explicit payer consent for ACH or wire transfers, reducing disputes. It supports audit trails and internal controls for treasury, accounting, and compliance teams without altering primary contract terms.

Why Parties Use a Miscellaneous EFT Addendum

Who completes the Miscellaneous EFT Addendum and why

Organizations and individuals attach this addendum when they need a formal electronic payment authorization tied to a contract, invoice, or settlement.

  • Treasury and Accounts Payable teams: Standardize vendor payment instructions and reduce check processing time.
  • Vendors and Contractors: Provide irrevocable bank routing and account details for recurring or one-time settlements.
  • Healthcare and Benefits administrators: Record patient or payee EFT consent while documenting handling of sensitive payment data.

Collecting completed addenda from authorized signers ensures consistent payment setup, supports reconciliation, and helps meet regulatory or internal audit requirements.

Core elements to include in a professional EFT addendum

A complete addendum balances operational detail with legal clarity so banks, payers, and payees can act without ambiguity.

Step-by-step: completing the Miscellaneous EFT Addendum

Follow these steps in order to collect a valid authorization and set up bank routing with your treasury or payments provider.

  • 01
Gather documents: Collect contract reference, voided check, and W-9 to verify entity and account details.
  • 02
Enter payment data: Complete banking fields, transaction scope, limits, and effective date exactly as the bank requires.
  • 03
Verify signer authority: Confirm the signer has corporate authority; attach board resolution or power of attorney if required.
  • 04
Store and route: Send the signed addendum to treasury or payment operations and retain an audit-ready copy for reconciliation.

Design workflows that capture bank data securely, enforce required fields, and route authorizations to the correct approvers.

Voided Check Upload Required; PDF or image accepted
Signer Authentication Email + SMS OTP or SSO per risk profile
Routing Rules Auto-route to AP/Treasury after signature

How the signed addendum is processed and where to submit it

A typical processing flow routes the signed addendum to payments operations, the originating contract folder, and the bank setup team.

  • Submit to AP: Send the executed addendum to Accounts Payable for invoice matching and schedule setup.
  • Send to Treasury: Treasury configures bank instructions, limits, and payment schedules in the banking portal.
  • Upload to Bank: Provide voided check or bank letter to the bank per their onboarding checklist.
  • Archive: Store the executed file and audit trail in secured records for reconciliation and audit.

Digital signing and submission considerations

Use secure eSignature platforms that capture an audit trail, support encrypted file storage, and allow attachments such as voided checks.

  • File formats: PDF, DOCX
  • Authentication: Email + SMS OTP, SSO
  • Integrations: ERP, treasury, and cloud storage connectors

Ensure your chosen platform supports required compliance needs (e.g., HIPAA BAA if payment details accompany protected health information) and provides tamper-evident signed PDFs and exportable audit logs.
