What the Miscellaneous I-200 Form Is and when it applies

The Miscellaneous I-200 Form is a multipurpose administrative form used to record or request information not covered by standard agency forms. It typically captures party details, a brief description of the transaction or request, dates, and authorizing signatures. Because the title and exact layout vary by issuing agency, organizations use this form where a short, structured written record is required but no dedicated statutory form exists. Proper completion ensures accurate routing, consistent recordkeeping, and reduced processing delays across agencies and internal teams.

Why accurately completing the Miscellaneous I-200 Form matters

A complete I-200 reduces processing errors, preserves legal evidence of the transaction, and clarifies responsibilities. Accurate entries support audit trails, help meet retention requirements, and minimize follow-up requests that slow approvals.

Who typically completes and reviews the I-200 Form

The I-200 is commonly used by a mix of administrative staff, managers, and external parties who need a short written record or authorization.

  • Administrative staff processing requests and routing documents internally.

  • External contractors or vendors supplying identifying information or consent.

Final reviewers ensure signatures, dates, and required attachments are present before filing or digital submission to preserve compliance and traceability.

Two real-world I-200 examples to illustrate usage

These examples show common situations where a miscellaneous form serves a clear administrative purpose.

Vendor Onboarding

A procurement team uses an I-200 to record vendor contact details and tax classification when no dedicated vendor form is available

  • Captures company name, payment terms, W-9 reference
  • The completed I-200 is attached to the vendor file and routed to AP and procurement for verification, accelerating onboarding and preventing duplicate vendor records.

Internal Authorization

A department head submits an I-200 to document an ad hoc budget reallocation under an existing grant

  • Summarizes amount, justification, and approval authority
  • Finance uses the I-200 as the administrative record, attaches backup, and archives it in the grant folder for audit readiness.

Essential data elements and security considerations

Names: Full legal names
Dates: MM/DD/YYYY format
Contact: Address, email, phone
Signatures: Signer name, title, date
