What the Miscellaneous MCS-150 Form Is and Why It Matters

The Miscellaneous MCS-150 Form is the Federal Motor Carrier Safety Administration's Motor Carrier Identification Report used to register or update a motor carrier's USDOT-related information. Carriers file the MCS-150 when they obtain a USDOT number and use it to report business name, physical and mailing addresses, contact details, types of operation, and fleet size. Accurate MCS-150 data supports regulatory oversight, roadside inspections, and insurance underwriting; maintaining current records helps ensure compliance with FMCSA registration requirements and avoids administrative follow-ups.

Why Completing the Miscellaneous MCS-150 Form Correctly Matters

Completing the MCS-150 accurately keeps your USDOT record current, supports compliance reviews, and minimizes costly follow-up from regulators or carriers' insurers. Accurate submissions reduce operational interruptions and clarify jurisdictional oversight under FMCSA rules.

Which organizations and roles commonly complete the MCS-150

Primary users range from owner-operators to fleet compliance teams and third-party registration services preparing carrier records.

  • Owner-operators updating their USDOT and contact data for single-truck operations.
  • Small- and medium-fleet safety managers tasked with biennial updates and data accuracy.
  • Third-party compliance vendors preparing registration updates and standing in for the motor carrier.

Each user should verify legal entity names and contact details before submitting to avoid rejections or follow-up correspondence.

Delayed Confirmation: Operational uncertainty while FMCSA reconciles records

Common mistakes to avoid when preparing the form

  • Submitting an incorrect USDOT number creates duplicate records and delays reconciliation with FMCSA.
  • Using a DBA instead of the legal entity name prevents proper linkage to safety and insurance files.
  • Providing a P.O. box for the physical address can trigger follow-up since location-based inspections rely on a street address.
  • Skipping verification of fleet counts and operation type can lead to misclassification and increased inspection attention.
