What the Miscellaneous No Testing Form Is

The Miscellaneous No Testing Form documents a decision that no diagnostic, laboratory, or compliance testing will be performed for a specified person, sample, item, or process. It records the parties involved, the scope and reason for not testing, relevant dates, and any follow-up instructions. Organizations use this form to create an auditable record that explains a deviation from standard testing protocols, supports regulatory transparency, and reduces later disputes about why testing was omitted.

Why this form matters

Using a standardized No Testing Form creates a clear, dated record of the rationale and authority for skipping required or optional tests, helps meet internal audit and compliance needs, and reduces confusion when multiple teams manage testing decisions.

Why this form matters

Who typically completes and reviews this form

Departments and roles vary by setting; common completors and reviewers are listed below.

  • Quality Assurance Managers responsible for documenting exceptions and audit trails across processes.
  • Occupational Health Staff who record clinical or workplace decisions not to perform specific tests.
  • Project Leads or Site Supervisors who authorize or log non-testing for materials, samples, or checks.

Include a reviewer signature and contact information so the decision can be verified later if questions arise.

Common preparation errors to avoid

  • Using vague language for the reason (e.g., 'not required') instead of citing the specific policy, risk assessment, or prior result that justifies no testing.
  • Failing to record the authorizer's full name and contact information, which delays audits and verification requests during inspections.
  • Not attaching supporting documentation such as prior test reports, chain-of-custody notes, or approval emails, leaving the decision unsupported.
  • Storing the completed form only in email without a permanent archive or version control, risking loss or inconsistency during audits.
