Identification

Machine model, serial number, manufacturer, date of manufacture, and location. Clear identification links the record to warranty, service history, and asset registers used by procurement and maintenance teams.

Inspection

Structured inspection checklist covering bearings, rollers, controls, guards, and lubrication. Include pass/fail fields, severity notes, photos, and required corrective action with due dates for repairs.

Maintenance

Log routine service, parts replaced with part numbers, technician labor hours, applied torque/settings, and next scheduled maintenance date to support preventive programs and cost entries.

Ownership

Document current owner, purchase or transfer date, purchase price if relevant, purchase order number, and contact details for the owner or custodian and associated contract references.

Compliance

Record regulatory checks, applicable standards, and any required certifications. Note HIPAA, OSHA, or industry-specific compliance steps when handling regulated materials or environments and attach supporting certificates or test reports.

Penalties and operational risks of incorrect records

Warranty Void: Missing serial or date
Regulatory Fines: HIPAA or OSHA exposure
Contract Disputes: Ambiguous ownership details
Operational Risk: Unrecorded defects cause downtime
Tax/Audit Issues: Incomplete records hinder audits
Signature Invalidity: Missing consent or proof

Common preparation mistakes to avoid

  • Failing to record the exact serial number or manufacturer details, which can break warranty claims, hinder parts ordering, and create ownership disputes between departments.
  • Using inconsistent date formats or omitting effective dates, causing ambiguity about when maintenance obligations or liabilities begin and complicating compliance reporting.
  • Skipping photographs or failing to attach invoices and parts lists, which removes corroborating evidence needed for insurance claims and supplier warranty work.
  • Relying on handwritten, illegible entries or unsigned forms that cannot be authenticated electronically, increasing the likelihood of rejection during audits.
